He was born in New York City on January 10, 1943. His parents, Anthony and Adeline, came from the Italian city of Naples. Massino grew up in the Maspeth area of Queens. He was known as "Big Joey" because he became overweight as an adult. He also faced health problems like diabetes and high blood pressure later in life.

Massino worked his way up through the criminal world. In the late 1960s, he was an associate in the Bonanno family. He led a crew that hijacked trucks to steal goods. He used a lunch wagon and a catering company called J&J Catering to hide his illegal work.

He worked very closely with his brother-in-law, Salvatore Vitale. Massino also became friends with another famous mobster named John Gotti. These early jobs helped him gain money and influence.

He fought hard to gain control of his crime family. In 1979, Massino helped his mentor, Philip Rastelli, remove a rival named Carmine Galante. This made Massino a more important leader.

In 1981, Massino helped kill three other leaders to stop them from taking power. He also arranged the death of Dominick Napolitano in 1981. This happened because Napolitano had been tricked by an undercover FBI agent named Joseph Pistone. Massino became the most powerful Mafia leader in the country by the year 2000.

Massino was a very unique leader during his time. Many other top bosses were in prison, but he was not. Because of this, people called him "The Last Don."

He spent time in prison for a crime involving labor racketeering in 1986. While he was away, Rastelli died and Massino took over as the official boss in 1991. He worked to rebuild the family after many years of trouble. He stayed in power until the law finally caught up to him in 2004.

In 2004, Massino was convicted of crimes in a large court case.

He faced a very serious punishment, including the death penalty. To avoid this, he made a big choice to turn state's evidence. This means he agreed to tell the truth to the government about his former partners. He testified against people like his former underboss, Salvatore Vitale. Because of his help, he was sentenced to life in prison in 2005. He later helped the government again in 2011 to convict another leader named Vincent Basciano.
