A commissioner is the highest-ranking official in a professional sports league. Bettman was tasked with several major goals when he took office. These included selling the game to the United States market. He also had to end labor unrest and complete expansion plans. Finally, he worked to modernize the views of the league's ownership ranks.

Before leading the NHL, Bettman worked for the National Basketball Association (NBA). He joined the NBA in 1981 to work in marketing and legal departments. He eventually rose to the position of third in command. In this role, he served as general counsel and senior vice president. One of his major achievements was developing the soft salary cap system. This system was implemented by the NBA in 1983. The league continues to use this specific financial structure today.

Bettman's leadership of the NHL has been defined by massive expansion. When he started, the league had 24 teams. He led efforts to grow the league's footprint across the United States. He focused heavily on the Sun Belt, which is the southern region of the country. This strategy added teams like the Nashville Predators in 1998 and the Atlanta Thrashers in 1999. By the 2021–22 season, his efforts helped bring the total to 32 teams.

Recent expansions include the Vegas Golden Knights in 2017 and the Seattle Kraken in 2021.

This expansion strategy caused significant controversy among hockey fans. Many people in Canada and the Northern United States felt the league was abandoning traditional markets. Critics even accused Bettman of having an "anti-Canadian" agenda. This was due to the relocation of teams from Quebec City and Winnipeg. Some fans felt he refused to help stop these moves. To address this, Bettman championed a Canadian assistance plan. This was a revenue sharing agreement. It used money from American teams to support four small-market Canadian teams. These teams were Calgary, Edmonton, Ottawa, and Vancouver.

Bettman also managed several difficult labor stoppages, known as lockouts. A lockout occurs when owners prevent players from working during a dispute. The 1994–95 lockout lasted 104 days and shortened the season to 48 games. The most severe event was the 2004–05 lockout. This disagreement resulted in the cancellation of the entire NHL season. This made the NHL the first North American professional sports league to do so.

After intense negotiations, the sides eventually agreed to a hard salary cap. This cap was linked to a fixed percentage of league revenues.

Managing the league also required complex organizational changes. During the late 1990s, the NHL changed its divisional alignment. The league moved from four divisions to six divisions. Each division contained five teams. This caused issues for teams in different time zones. For example, the Dallas Stars were in the same division as California teams. This made scheduling difficult for television broadcasts. To fix this, Bettman brokered negotiations to revert to a four-division alignment. This new structure was implemented for the 2013–14 season.

Bettman has received many professional honors throughout his career. In 2014, he was named "sports executive of the year" by two different journals. He was elected to the Hockey Hall of Fame in 2018. He was also inducted into the International Jewish Sports Hall of Fame in 2016.
