He was born Lev Milstein on September 30, 1895, in Kishinev, Russia. His family was wealthy and held a distinguished place in their community. Even though his parents wanted him to study engineering in Germany, he loved the theater. He eventually left his studies behind to follow his dream of acting and drama. In 1913, he moved to the United States to start a new life. He arrived in Hoboken, New Jersey, shortly after his eighteenth birthday.

Life in America was a hard job at first for the young man. He worked as a janitor, a salesman, and a worker in a lace factory. In 1915, he found a job as a photography assistant. This work led him to join the Army Signal Corps in 1917. While in the army, he learned many important skills for making films. He practiced aerial photography and helped make training films for the military. He even worked on editing footage from combat situations. These early lessons gave him a strong start in the world of movies.

After he left the army in 1919, he became a U.S. citizen. He changed his name from Milstein to Milestone at the suggestion of a judge. He then moved to Hollywood to find work in the film industry. He started with very small roles, such as an assistant editor. He once worked as a card dealer at a gambling venue to pay his bills. Through hard work, he moved up from being a cutter to a general assistant. By 1920, he was working closely with director Henry King at Pathé Exchange.

Milestone became known for his many different skills in Hollywood. He worked as an editor, a writer, and even a gag writer for comedians. People called him a "film doctor" because he was great at fixing movies. This meant he could step in and make a film work much better. In 1925, he made his debut as a director with a film called Seven Sinners. He also directed The Caveman in 1926, which earned him a lot of praise. His talent for switching between comedy and suspense made him a rising star.

His career reached great heights during the silent film era. In 1927, he directed Two Arabian Knights, which is considered one of his best works. This film won him an Academy Award for Best Director. He also directed the famous movie All Quiet on the Western Front in 1930. Other notable films in his long career include Of Mice and Men and Ocean's 11. He remained a major part of the movie world for many decades. His journey from a student in Russia to a Hollywood legend is truly amazing.
