The river is part of a large area called the Fish to Tsitsikama Water Management Area. Its name is actually a bit of a mistake. Long ago, people used a Dutch name that meant Great Fish River for a different stream nearby. This river is actually the Fish River, but it is often called the Great Fish River to tell it apart from another river in Namibia. The area near its mouth is known as the Sunshine Coast.

Watching the river flow is like seeing a long journey. It starts east of Graaff-Reinet and runs through a town called Cradock. The Tarka River joins it on the left side. Then, the river makes a zig-zag turn to reach Cookhouse. It meanders down a steep slope called an escarpment near Grahamstown. Finally, it makes a straight run to its mouth near Seafield. The river flows into the warm waters of the Indian Ocean.

This river has a very long history with many different people. In the late 1480s, a man named João Infante visited the river. During the 1800s, the river was a border for the Cape Colony. It was a place of many battles during the Xhosa Wars. Soldiers used a small bay called Waterloo Bay to land supplies. You can still find traces of the past there today. Some people even found old items while building a golf course! There is also a small cemetery where soldiers were once buried.

Life along the river is full of many living things. You might see a clever vervet monkey looking for food. There are over 135 kinds of birds, including the giant kingfisher. Some special trees, like the red and white milkwood, are protected. The river is also home to 26 types of snakes. Divers can go under the water to see old shipwrecks. They might see the remains of ships named the Catherine or the Justina. These ships sank near the river mouth in the 1840s.

Today, the river is a place for fun and work. People join the Fish River Canoe Marathon, which is a big two-day race. Others enjoy swimming, golf, or walking in nature. A lighthouse called Great Fish Point stands near the coast to help ships. It was built in 1898 to keep sailors safe. Even though the river can be dry sometimes, engineers use a big tunnel to bring water from the Orange River. This helps keep the river flowing for the plants and animals that live there.
