This makes it the second-largest city in the Caribbean region. It is also the fifth-largest city in Colombia. The metropolitan area is the sixth-largest urban area in the country.

Human history in the Cartagena Bay area began thousands of years ago. Around 4000 BC, the Puerto Hormiga culture established a community here.

This is considered the first documented human community in modern Colombia. These people lived in a mild climate with abundant wildlife. They produced some of the oldest ceramic objects found in the Americas. Around 3000 BC, this culture began to decline. A later group called the Monsú culture emerged near Barú Island.

The Monsú people used pottery and practiced agriculture. They also relied on a diet of fish and shellfish. Later, various tribes from the Carib language family lived in the region. Some of these groups, like the Kalamari, lived on sandy islands. They built circular huts with tall roofs and defensive wooden palisades.

European contact led to the formal founding of the Spanish city. Pedro de Heredia arrived in the bay with three ships in 1533.

He founded the city on June 1, 1533. The name comes from Cartagena in Murcia, Spain. Heredia built the settlement on the site of the former Calamarí village. He led expeditions that brought back significant amounts of gold. By the early 1540s, the city became a major trade hub. Its position between the Magdalena and Sinú rivers was very strategic. This location allowed easy access to the interior of New Granada. The city became a key link between Spain and its overseas empire.

Cartagena played a massive role in the Spanish colonial economy. It was a primary port for exporting Bolivian silver to Spain. The city also operated under the asiento system for importing enslaved Africans. This made the city a center of political and economic activity. Because of this wealth, the city was a target for many attackers. In 1544, the French pirate Jean-François Roberval pillaged the city. Later, the pirate Francis Drake attacked in 1586 with 23 ships.

Drake burned 200 houses and the cathedral before leaving. In 1741, the city saw major combat during the War of Jenkins' Ear. These constant threats forced the Spanish to improve their defenses.

To protect the city, the Spanish built massive fortifications. Between 1614 and 1796, they constructed 11 kilometers of defensive walls.

These walls surrounded the historic center to stop pirate attacks. The Spanish also commissioned master plans for port defense. Bautista Antonelli designed schemes to protect Caribbean ports in the late 1500s. One specific defensive tower, San Felipe del Boquerón, was built in 1566. The city also features many historic religious buildings. The Convento de Santo Domingo is the oldest church in the city.

The city also hosted the Holy Office of the Inquisition. This institution was established in Cartagena in 1610. The Palace of the Inquisition was completed in 1770.

The Inquisition punished crimes like heresy, blasphemy, and witchcraft. During ceremonies called Autos de Fe, sentences were read in the main plaza. A total of 767 people were punished by this office. These punishments included fines, imprisonment, or death. The Inquisition remained active until independence in 1811. This period shows the complex social and legal structures of the colonial era.

Today, Cartagena is recognized globally for its preserved heritage. In 1984, UNESCO designated the walled city and its fortresses as a World Heritage Site.

Visitors come to see the historic architecture and the sea. The city remains a vital part of Colombia's maritime identity. It connects ancient indigenous roots with colonial history and modern industry. The transition from a defensive outpost to a major port is complete. Cartagena stands as a testament to centuries of global trade and conflict.
