Brain death is a very serious medical state. It means the brain has lost all function in a way that cannot be fixed. This loss is permanent and irreversible.

It is different from a coma or a persistent vegetative state. In those states, some parts of the brain still work. In brain death, the whole brain has stopped working. This includes the brainstem, which is the part that helps the body breathe. Without it, a person cannot breathe on their own without machines.

Doctors follow a careful way it works to find brain death. First, they check for any response to pain. They also check for cranial nerve reflexes, like if the eyes react to light. They look for the absence of spontaneous breathing. Sometimes, doctors use an EEG to check electrical activity. An EEG is a test that shows brain waves. In brain death, the EEG may show a flat line. They might also use a radionuclide scan to see blood flow.

A scan might show no blood flow in the brain, which is sometimes called a "hot-nose" sign.

Understanding this state has a long history. In the past, doctors decided death by looking for a heartbeat or breathing. As medical machines got better, they could keep bodies working even without breathing. This made the definition of death more complicated. In 1968, a report from Harvard Medical School helped define these states. Later, in 1981, a presidential commission in the United States helped create a "whole-brain" definition. This helped many states create clear laws about when a person is legally dead.

There are many important facts about how different countries handle this. Finland was the first European country to use brain death as a legal definition in 1971. In the United Kingdom, the Royal College of Physicians suggested a definition based on the brainstem in 1995. In 2020, an international group called the World Brain Death Project published new guidelines. They wanted to make sure doctors all over the world use the same standards.

These rules help make sure the diagnosis is always very accurate.

This topic links to how we help others through organ donation. Because life support machines can keep organs working, they can be used to help sick people. In some places like Spain or the UK, people are automatically donors. In other places, like most of the United States, families must give consent first.

It is important to know that some body movements can still happen. These are called the Lazarus sign. These movements come from the spinal cord, not the brain. They do not mean the brain is working again.
