Many different people have lived near or visited this mountain. The Lakota Sioux people have long called this area Pahá Sápa. They consider the Black Hills to be a sacred site. The Lakota had many names for the peak. One name was Hiŋháŋ Káǧa, which means "owl-maker." This name came from rock formations that looked like owls. Another name was Heȟáka Sápa, which means "elk black."

The mountain has a long and changing history. In 1855, an American named Lieutenant Gouverneur K. Warren named it Harney Peak. He named it after General William S. Harney. For many years, the Lakota people asked to change this name. They did not want to honor a man whose forces had fought their people. In 2016, a big change finally happened. The U.S. Board on Geographic Names voted to rename it Black Elk Peak. This honors a wise Lakota medicine man named Black Elk.

There are many interesting facts about the peak itself. In September 2016, surveyors used special tools to find its exact height. They found a nearby secondary peak called "McGillicuddy's Peak." This smaller peak is about 300 feet south of the lookout tower. The mountain has also had a post office in the past. It operated from 1936 to 1942 and again from 1945 to 1946. Some people say it was one of the highest post offices in the country.

Today, people visit the mountain to explore the outdoors. You can hike to the summit from places like Sylvan Lake. The trail from Sylvan Lake is about 7 miles long. It is the most popular and easiest path for hikers. At the top, you can see an old stone fire lookout tower. This tower was built in 1938 by the Civilian Conservation Corps. There is also a dam and a pumphouse nearby. These historic structures are listed on the National Register of Historic Places.
