Thiamine deficiency

life science Maturity 7-9 Next article →

Thiamine is a vitamin known as B1 that our bodies need to stay healthy. When a person does not get enough thiamine, it causes a medical condition. A severe and long-lasting form of this sickness is called beriberi.

The name beriberi may come from an 18th-century phrase meaning "I cannot, I cannot." This name describes the great weakness people feel when they are sick. It is a serious matter because thiamine helps our bodies turn food into energy.

This condition works by disrupting how our cells use energy. Thiamine turns into a helper called thiamine pyrophosphate, or TPP. This helper is part of a central pathway called the citric acid cycle. This cycle is the main way our bodies manage sugars, fats, and proteins. Without enough TPP, the cycle is disrupted. This can stop the body from making important brain signals like glutamic acid and GABA.

History shows that beriberi has been known for thousands of years in Asia. It became more common in the late 1800s. This happened because people began processing rice more often. Processing rice can remove the thiamine that is usually found in the grain. Today, thiamine deficiency is rare in most developed parts of the world. However, it is still common in sub-Saharan Africa and in refugee camps.

There are several different types of beriberi. Dry beriberi affects the nerves and can cause numbness or trouble walking. It can also lead to brain issues like Wernicke's encephalopathy or Korsakoff syndrome. Wet beriberi affects the heart and the way blood moves. It can cause a fast heart rate and swelling called edema.

There is also gastrointestinal beriberi, which was recognized in 2004. This type affects the digestive system and causes stomach pain. Finally, infantile beriberi affects babies, often because their mothers did not have enough thiamine.

Doctors can find this condition by looking at symptoms or testing urine and blood. They might check for high levels of lactic acid. One way to test is by looking at how an enzyme called transketolase works in red blood cells.

Once found, the treatment is usually thiamine supplements. These can be taken by mouth or through an injection. Most people feel much better within a few weeks of starting treatment. To help everyone, many places add thiamine to food to prevent the sickness from starting.
