Pellagra

life science Maturity 11-13 Next article →

Pellagra is a serious disease caused by a lack of niacin. Niacin is also called vitamin B3. This vitamin helps the body perform many important jobs. Without enough niacin, the body cannot make a helper called NAD.

This lack of help can cause many different health problems. If it is not treated, pellagra can lead to death. It often affects people living in poverty or those with poor diets.

This sickness works in two main ways. Primary pellagra happens when a person does not eat enough niacin or tryptophan. Tryptophan is an amino acid found in meat, eggs, and peanuts. The body can actually use tryptophan to make its own niacin.

Secondary pellagra happens when the body cannot use the niacin it eats. This might happen because of certain medicines or other diseases. Some conditions can even stop the body from absorbing nutrients properly.

Doctors look for specific signs to find pellagra. One sign is a skin rash called a Casal collar. This rash usually appears around the neck.

Other symptoms include diarrhea, hair loss, and a red, swollen tongue. It can also cause mental confusion or trouble with coordination. To confirm the diagnosis, doctors might test a person's urine. They often see quick improvements after giving the person nicotinamide. This is a type of niacin that helps the body heal.

History shows us how much we have learned about this disease. In 1735, Gaspar Casal first described the skin effects in Spain. Later, in the early 1900s, pellagra became a huge problem in the American South. Between 1906 and 1940, more than 3 million Americans were affected.

Dr. Joseph Goldberger studied the disease in places like orphanages. He discovered that adding meat, milk, and eggs to diets helped prevent it. This proved that the sickness was linked to nutrition rather than germs.

Understanding pellagra helps us see how food and culture connect. For example, many cultures rely on corn as a main food. Native American groups used a process called nixtamalization to prepare corn. They treated the grain with an alkali solution like lime. This simple step makes the niacin in the corn easy for the body to use. Without this step, eating mostly corn can lead to pellagra. This shows how a way of preparing food can keep people healthy.
