Macrinus faced many hard jobs when he took power. The empire was at war with several lands. These included Parthia, Armenia, and Dacia. Rome's money was also in a bad state. Caracalla had spent a lot of money on the army. Macrinus tried to fix this with new rules. He made peace with the Parthians in 217. He also made peace with Armenia. He even made peace with the Dacians.

To fix the money, Macrinus changed how much soldiers were paid. He let older soldiers keep their high pay. However, he reduced the pay for new recruits. He also made the silver in coins more pure. These changes were meant to bring back stability. But these choices made the Roman military very unhappy. The soldiers felt their future pay was at risk. This loss of support made his position weak.

A woman named Julia Maesa started a rebellion. She wanted her grandson, Elagabalus, to be the new emperor. Elagabalus was only fourteen years old. The soldiers joined this revolt. Macrinus fought a big battle at Antioch in June 218. He lost the battle and had to flee. He tried to reach Rome, but he was caught. His son, Diadumenianus, was also caught and killed. After they died, the Senate removed their names from all records.
