His life took a major turn during his teenage years.

In March 1940, a 14-year-old Sathya was stung by a scorpion. He lost consciousness for several hours during this event. When he regained consciousness, his behavior changed significantly. He displayed alternating states of laughter, weeping, eloquence, and silence. He also began singing Sanskrit verses without prior formal training. Doctors at the time diagnosed his behavior as hysteria.

On 23 May 1940, Sathya made a major proclamation. He told his family that he was the reincarnation of Shirdi Sai Baba. Shirdi Sai Baba was a famous saint from Maharashtra. This was the moment he became known as Sathya Sai Baba. He stated his mission was to re-establish Dharma, or righteousness. He also aimed to motivate love for God and service to humanity.

Many followers attributed miraculous abilities to him. They claimed he could materialize Vibhuti, which is holy ash. He was also said to produce rings, necklaces, and watches. Some believed he possessed clairvoyance and the ability of bilocation. Bilocation is the claim of being in multiple places at once. Others believed he possessed omniscience and omnipotence. However, he faced various accusations of fraud and sleight of hand. Despite these claims, he was never charged with any crime.

Sathya Sai Baba dedicated much of his life to large-scale charity. In 1954, he established a free general hospital in Puttaparthi. In 1995, he started a massive drinking water project. This project aimed to serve 1.2 million people in the Rayalaseema region.

He also built several temples in major Indian cities. These included the Sathyam Mandir in Mumbai and Shivam Mandir in Hyderabad. His work focused on meeting the basic needs of the poor.

His later years included several health challenges and security incidents. In 1963, he reportedly suffered a stroke and four heart attacks. He claimed to have healed himself in front of his followers. In 1993, an assassination attempt occurred at his residence. Four men entered his home and injured two assistants. The incident ended when police shot the four men. In 2003, he suffered a fractured hip after an accident involving a student.

Sathya Sai Baba died on 24 April 2011 at age 84. He had previously predicted he would live to age 96. Some devotees explained this using a lunar star-based calendar. They calculated his age based on 324-day years. Under this system, he had completed 95 years and 54 days. Approximately 500,000 people attended his burial ceremony.
