He served as the fifth president of the nation. He was born in the city of Guaratinguetá in 1848. He studied to be a lawyer in São Paulo. Early in his career, he worked as a councilman and a prosecutor. He also believed that education should be free for everyone. Later, he served as the president of the province of São Paulo. He was a member of the conservative party during that time.

In 1902, Alves became the president of Brazil.

He wanted to make the capital city, Rio de Janeiro, much cleaner. At that time, the city had a very large population. Many families lived together in single houses. Rats and mosquitoes were everywhere in the city. These pests helped spread dangerous diseases like yellow fever and the plague. Alves believed that poor sanitation made the whole country weak. He wanted to fix the city to improve its image to the world.

To help the city, Alves hired experts for big jobs. He appointed Oswaldo Cruz to lead public health efforts. He also chose Pereira Passos to be the Mayor of Rio de Janeiro. Cruz worked hard to fight diseases. In 1903, there were 584 deaths from yellow fever. By 1904, that number dropped to only 53. By 1906, the plague had almost completely disappeared. However, things became difficult in November 1904. The government made smallpox vaccines mandatory, which caused the Vaccine Revolt among the people.

While Cruz worked on health, Mayor Pereira Passos worked on the city itself. He wanted Rio de Janeiro to look like modern cities in Europe. He extended the port and built new, wide avenues. This work also meant many families had to move to the suburbs. The government used money from rubber sales to pay for these changes. At that time, Brazil produced 97% of all the world's rubber. The city also saw new things like the first automobiles. Rules were made to keep streets clean and free of livestock.

Alves also helped Brazil settle borders with other lands. His minister, José Maria da Silva Paranhos, helped resolve disputes with Bolivia and others. In 1903, Brazil added the area of Acre to its territory. This happened through the Treaty of Petrópolis. Alves was elected president again in 1918. He won with over 99% of the votes. Sadly, he became very ill during the Spanish flu pandemic. He died on January 16, 1919, before he could start his new term. Delfim Moreira became the next leader.
