Retainer (orthodontics)

life science Maturity 11-13 Next article →

When teeth are straightened with braces, they do not always stay in their new spots. There is a lifelong risk called relapse. This is when teeth try to return to their original positions. This happens because of pressure from soft tissues or the recoil of periodontal fibres. Even a person's continued growth can cause changes.

A retainer is a special device used to hold teeth in place. These tools help the surrounding tissues adapt to changes in the bone. This helps minimize movement after orthodontic treatment is finished.

There are different ways these tools work. Removable retainers can be taken in and out of the mouth. Some people wear them part-time or full-time.

The Hawley retainer is a very well-known type. It uses metal wires that go around the front teeth. These wires are held by an acrylic baseplate on the roof of the mouth. This type is very strong and can be adjusted by a dentist. It can even have extra parts added to fix specific tooth rotations.

Another common type is the vacuum-formed retainer, or VFR. These are made of clear plastic like polypropylene or PVC.

They are made using a thermoforming process. This means heat and a vacuum help shape the plastic over the teeth. Because they are clear, they are almost invisible when you wear them. Some VFRs cover all the teeth, while others are just clip-on styles. They are often easier to make and can be more economical than other types.

Some people use fixed retainers that stay in the mouth. A dentist bonds a wire to the back of the teeth. They use a material called composite to glue the wire in place.

This is helpful for people with severe rotations or other stability issues. These can be made with smooth wire or a flexible spiral wire. While they do not rely on a person to remember to wear them, they are harder to clean. Patients must be careful to prevent plaque from building up around the wire.

Keeping a retainer clean is a very important job. If they are not cleaned, they can trap food and cause cavities.

You can clean removable ones with a toothbrush and mild, fragrance-free soap. It is important to avoid very hot water because it can make the plastic shrink. When you are not wearing a removable retainer, keep it in a dry container. Newer tools like SMART Retainers even use mobile apps to help people remember to wear them. These apps can even send notifications to parents to help children stay on track.
