His career spanned several decades and multiple countries. He represented the national teams of both Spain and the Philippines. Alcántara is remembered as one of the greatest strikers in football history. FIFA officially recognized him as a top 20th-century player. They also named him the greatest Asian male footballer of all time.

His journey began in Concepcion, Iloilo, in the Philippines. He was born in 1896 to a Spanish-Filipino family. His father was a Filipino criollo Spaniard from Manila. His mother was a Filipina mestiza of mixed Spanish origin. When he was three, his family moved to Barcelona, Spain. This move happened because of the Philippine Revolution. In Barcelona, he joined a club called FC Galeno. This team was composed mostly of medical students. They practiced in the courtyard of a local hospital.

Alcántara's rise to fame was incredibly rapid. In February 1912, Joan Gamper scouted him for FC Barcelona. He made his first-team debut at only 15 years old. He played against Catalá SC in the Campionat de Catalunya. During this match, he scored three goals. This made him the youngest player to score for the club. It also made him the youngest hat-trick scorer in the world. That specific record stood for 84 years.

His career included a significant period in the Philippines. In 1916, his parents moved the family back to the islands.

He eventually returned to Spain to receive proper medical treatment.

Back in Barcelona, Alcántara entered a golden era for the club. He was a powerful player known for his intense shots. In 1922, he earned the nickname "El Rompe Redes." This means "the net breaker" in Spanish. He received this name after a shot broke a goal net. He helped Barcelona win many major trophies. He was a Lieutenant in the Brigade Legionary Black Arrows. This was a volunteer corps directed by Benito Mussolini. He participated in military operations in Aragon and Catalonia.

In his later years, he returned to football management. In 1951, he coached the Spanish national team for three games. He also served as President of the FC Barcelona Player Association in 1959.
