Ferrigno's journey began with a major physical obstacle during his childhood. Shortly after birth, a series of ear infections caused him to lose 75% to 80% of his hearing. This condition was not officially diagnosed until he was three years old. The resulting hearing loss and speech impediment led to him being bullied by peers. To defend himself, he became obsessed with the idea of power and strength. He began weight training at age 13, using homemade equipment. Because he could not afford professional weights, he used a broomstick and cement-filled pails.

His dedication to bodybuilding led to several prestigious professional titles. After graduating high school in 1969, he won the IFBB Mr. America title. He went on to win two consecutive IFBB Mr. Universe titles in 1973 and 1974. During this era, he lived in Columbus, Ohio, and trained alongside Arnold Schwarzenegger. He also competed in the 1977 World's Strongest Man competition, finishing fourth out of eight competitors. The 1977 documentary Pumping Iron, which featured his attempt to beat Schwarzenegger, brought him widespread fame. In the early 1990s, he returned to the sport to compete for Mr. Olympia titles. He finished 12th in 1992 and 10th in 1993.

Acting became Ferrigno's primary career following his success in bodybuilding. In 1977, he was cast in the title role of the CBS series The Incredible Hulk. He played the character until 1981, though some episodes aired as late as 1982. During this time, he formed a close bond with actor Bill Bixby. Ferrigno viewed Bixby as a mentor and a father figure. He also appeared in several television movies alongside Bixby. His acting roles expanded into the fantasy genre with the 1983 film Hercules. Despite mixed reviews for the film, critics like Gary Allen Smith praised his physical aesthetics.

Ferrigno's connection to the Hulk character extended far beyond the original series. He made cameo appearances as a security guard in the 2003 film Hulk and the 2008 film The Incredible Hulk. In the 2008 version, he voiced the Hulk, a role he continued to perform in various Marvel Cinematic Universe films. He provided his voice for the character until the 2015 film Avengers: Age of Ultron. He also appeared as himself in the sitcom The King of Queens from 2000 to 2007. In these appearances, he and his wife, Carla, were depicted as neighbors to the main characters.
