Her path to stardom began with modeling. In January 2003, she created her first modeling portfolio. She personally described this initial attempt as a "disaster." However, she continued with more photo shoots and ramp shows. This led to a second portfolio in 2004. This new work secured her advertisements for brands such as Electrolux and Emami Talc. A specific advertisement for Fair & Lovely, directed by Rakesh Roshan, provided the exposure she needed. This visibility resulted in many offers for feature film roles.

D'Cruz officially entered the film industry in 2006. Before her debut, she took acting classes with Aruna Bhikshu. Her first feature film was the Telugu romantic-drama Devadasu. The filming process was emotionally difficult for her. She felt significant pressure and nearly decided to stop. Her mother provided encouragement through a long lecture at 3 am. The film became a major commercial success. It grossed approximately 140 million and won her the Filmfare Award for Best Female Debut – South.

Following her debut, D'Cruz became a leading figure in Telugu cinema. In 2006, she starred in the film Pokiri. This movie was a major success and became the highest-grossing Telugu film of its time. The film earned her a nomination for the Filmfare Award for Best Actress – Telugu. She continued this success with the 2008 film Jalsa. Jalsa was one of the highest-grossing films of that year. For her performance, she won the Santosham Award for Best Actress.

Throughout her career, D'Cruz has experienced both great success and fluctuations. While many of her films were major box office hits, others were considered poor grossers. Despite these variations, she remained one of the highest-paid actresses in South Indian cinema. She has also taken breaks from the industry, including a three-year sabbatical. In 2024, she returned to the screen with the films Unfair & Lovely and Do Aur Do Pyaar. Currently, she continues to be a recognized name in the industry. She is married to Michael Dolan, and they have two sons.
