A hospital ship is a specialized vessel designed to function as a floating medical treatment facility.

These ships are essential because they bring medical care directly to people in areas where land hospitals are unavailable. Most hospital ships are operated by national navies to support military forces near war zones. However, some are also run by civilian non-governmental organizations (NGOs) to provide humanitarian aid. Because of their mission, these ships are protected by international laws that designate them as non-combatant vessels.

Historically, the concept of the hospital ship has changed significantly over many centuries. In ancient times, the Athenian Navy had a ship named Therapia, and the Roman Navy had one named Aesculapius. During the Renaissance, the Spanish used a large flagship called a capitana as a hospital ship during the 1535 conquest of Tunis. By the 1600s, the British began using ships like the Goodwill to house sick sailors. These early experiments often lacked organization, and patients were sometimes left behind at ports when ships were reassigned.

In the mid-seventeenth century, the Royal Navy began formally designating specific vessels as hospital ships. These were often older ships or hired merchant vessels that underwent physical changes to improve medical care. Crews removed internal bulkheads, which are the walls inside a ship, to create more open space for patients. They also cut additional ports through the deck and hull to improve internal ventilation.

The medical staff typically included a surgeon and several surgeon's mates. While they provided basic supplies like bandages and soap, the quality of life was difficult. Surgeons reported that food was often rotten and bread was so hard it could injure patients' mouths.

The modern era of hospital ships emerged during the Crimean War in the 1850s. During the Siege of Sevastopol, nearly 15,000 wounded troops needed transport from the port of Balaklava. This era saw the introduction of steamships like HMS Melbourne and HMS Mauritius, which were the first to have genuine medical facilities. These ships featured spacious accommodations and specialized operating theatres for surgery.

By the late 1800s, technology advanced further, allowing ships to carry anesthetics like chloroform and antiseptics like carbolic acid to prevent infection.

During the two World Wars, the use of hospital ships expanded to a massive scale. Many large passenger liners were converted to serve as medical vessels. For example, the RMS Mauretania served as the HMHS Mauretania during World War I.

By the end of the First World War, the British Royal Navy had 77 such ships in service. In the United States, the Navy commissioned the USS Relief in 1921 as its first purpose-built hospital ship.

During World War II, the U.S. Navy and Army operated ships with different roles. Navy ships were fully equipped to receive casualties directly from battlefields, while Army ships acted as transports to move patients from forward areas to rear hospitals.

To ensure safety, hospital ships must follow strict rules established by international agreements like the Hague Convention X of 1907. These rules require ships to be painted white so they are easily seen. Military hospital ships must display a green band, while ships operated by relief societies must show a red band. They must also fly a red cross flag alongside their national flag.

According to international law, these ships must provide medical help to wounded personnel of all nationalities. They cannot be used for any military purpose, and attacking a ship that follows these rules is considered a war crime.

Today, hospital ships continue to play a vital role in both military and humanitarian work. Some ships, like the USNS Comfort and USNS Mercy, are famous examples of modern naval medical vessels. Others, such as those operated by the charity Mercy Ships, belong to civilian agencies and focus on humanitarian missions.
