Hemorrhoid

life science Maturity 11-13 Next article →

Hemorrhoids, which some people call piles, are part of our normal anatomy. They are vascular structures found in the anal canal. These structures act like soft cushions to help us control our bowel movements.

They become a disease only when they swell or become inflamed. When this happens, doctors often use the word hemorrhoid to describe the condition itself. About 50% to 66% of people will have problems with them at some point.

These cushions work by using special blood vessels called sinusoids. These vessels are part of a group known as the hemorrhoidal plexus. They are made of connective tissue and smooth muscle.

These cushions help with anal closure pressure when we are at rest. When a person bears down, the pressure in the abdomen grows. This causes the cushions to increase in size to help maintain closure. This is a natural way the body manages pressure during a bowel movement.

There are two main types of hemorrhoids. Internal hemorrhoids start above the pectinate line, which is a dividing line in the canal. They are covered by a lining that lacks pain receptors, so they are often painless. They may cause bright red bleeding during a bowel movement.

External hemorrhoids occur below that same line. These are covered by skin that is very sensitive to pain and temperature. They can cause swelling, itching, or pain if they become thrombosed, which means a blood clot has formed.

While the exact cause is unknown, many things can increase pressure in the abdomen. This pressure is a major factor in why they swell. Constipation and diarrhea can both lead to this issue. Sitting on the toilet for long periods or straining can also be a cause.

Pregnancy is another common time for them to appear. This happens because the fetus puts pressure on the abdomen. Hormonal changes and the birth itself also increase pressure in that area.

Doctors can help by looking at the area during a physical exam. They might use an anoscopy, which is a small tube with a light.

Many people find relief by eating more fiber and drinking plenty of fluids. This helps keep bowel movements regular. Some people might need a small procedure like rubber band ligation. This uses an elastic band to cut off the blood supply to the hemorrhoid. Most people find that their symptoms get better after a few days of rest.
