A heart transplant is a major surgery used to save lives. It is for people with end-stage heart failure. This happens when other medical treatments no longer work. Doctors take a healthy heart from a recently deceased donor. Most donors are people who have experienced brain death.

This procedure is not a cure for heart disease. Instead, it is a way to improve how a person lives. It helps them live for a longer time. On average, people live for 15 years after the surgery.

There are two main ways doctors perform this surgery. The most common way is called an orthotopic procedure. In this way, doctors remove the sick heart and replace it. The new heart takes the old heart's place. A less common way is a heterotopic transplant. Some people call this a "piggyback" transplant. In this version, the surgeon leaves the old heart inside the body. The new donor heart is then placed next to it to help.

Scientists have dreamed of this for a long time. In 1907, Simon Flexner wrote that surgery might one day replace human organs. In 1964, James D. Hardy tried a different kind of surgery. He used a chimpanzee heart to help a dying patient. This is called a xenotransplant, which means moving an organ between different species. The heart beat for 60 to 90 minutes before the patient died. Later, Christiaan Barnard performed the first human-to-human transplant in 1967. He used methods developed by surgeons Norman Shumway and Richard Lower.

Many important milestones happened in the years following the first transplant. In 1967, Adrian Kantrowitz performed the first pediatric heart transplant. This was a surgery for a child. In 1968, Norman Shumway performed the first adult transplant in the United States. That same year, a team in the United Kingdom performed their first one. In 1983, a drug called cyclosporine became widely used. This drug helped prevent the body from rejecting the new heart. In 1984, JP Lovette IV became the first successful pediatric transplant recipient.

Today, heart transplants are a regular part of medicine. About 5,000 transplants happen around the world each year. More than half of these take place in the United States. Vanderbilt University Medical Center in Tennessee is a very large center. In 2024, they completed 174 transplants. Scientists are even testing gene-edited pig hearts for humans. In 2022, David Bennett received one of these hearts in Maryland. This research helps because many more people need hearts than are available.
