Nagai’s career began with a profound personal turning point. Born on September 6, 1945, in Wajima, Ishikawa Prefecture, he was the fourth of five brothers. After his father died, he moved to Tokyo with his mother and brothers. During his years in a prep school, he suffered from a severe three-week illness. He was diagnosed with catarrh of the colon, which is an inflammation of the mucous membrane in the colon. Facing his own mortality, he decided to pursue manga to leave a legacy. Despite his mother’s opposition, he began working as a manga artist with help from his brother, Yasutaka.

His path to professional success involved significant perseverance. Early submissions to publishers were often rejected, sometimes because his mother secretly convinced them to refuse his work. However, his talent was eventually noticed by the magazine Weekly Shōnen Sunday. In 1965, he joined the studio of the famous artist Shotaro Ishinomori. During this time, he created a 19-year-old prototype for a story called Kuro no Shishi. This work grew from 15 pages to 88 pages over a full year. Although Ishinomori praised the effort, he noted that Nagai’s designs were too chunky and needed refinement.

Nagai made his professional debut in November 1967 with a short gag comedy titled Meakashi Polikichi. Shortly after, he achieved massive success with the series Harenchi Gakuen. This series was a major factor in the success of the new magazine Shōnen Jump. The magazine reached sales of more than one million copies per week because of his work. Harenchi Gakuen introduced eroticism to modern manga, which changed the industry forever. While it was a massive commercial hit, it also sparked intense social controversy. Many groups, including the PTA, protested the series for being vulgar and inappropriate for children.

These protests had a direct impact on Nagai’s creative direction. He was frequently called an "enemy of society" and a "nuisance" by critics. In response to the pressure, Nagai shifted his storytelling from nonsense gags to much darker themes. He transformed Harenchi Gakuen into a serious story about war and depicted violence in a bloody manner. This period also saw the creation of Abashiri Ikka and Guerrilla High, which parodied the conflict between youths and adults. This shift allowed him to move away from the gag comedy expectations held by his editors.

In the 1970s, Nagai successfully transitioned into more serious and complex genres. He wrote the science fiction one-shot Oni -2889 Nen no Hanran-, set in the year 2889. He also explored horror through series like Demon Lord Dante and the legendary Devilman. Devilman allowed him to fulfill his lifelong aspiration to write serious science fiction. To manage his growing business, he founded Dynamic Productions in April 1969 with his brothers. This company was essential for managing his contractual rights and royalties. Dynamic Productions was also a pioneer in industry standards, being one of the first to require formal signed contracts from publishers.

Nagai’s professional life has been marked by deep collaborations and lasting contributions. Ken Ishikawa, who became his regular partner and best friend, joined Dynamic Productions in 1970. Ishikawa assisted on many of Nagai's major works, including Harenchi Gakuen and Abashiri Ikka. Beyond his creative output, Nagai has contributed to the academic and professional community. In 2005, he became a Character Design professor at the Osaka University of Arts. Since 2009, he has served as a member of the Tezuka Osamu Cultural Prize's nominating committee. His career demonstrates how personal resilience and genre innovation can reshape an entire cultural landscape.
