Cholera is a serious infection in the small intestine.

It is caused by a type of bacterium called Vibrio cholerae. This germ can cause many different symptoms in people. Some people feel nothing at all from the infection. Others may experience mild sickness or very severe illness. The most famous sign is large amounts of watery diarrhea. This diarrhea can look like cloudy rice water. It may even have a fishy smell.

This sickness matters because it can spread very quickly.

The way the infection works is a step-by-step process. First, a person swallows the bacteria through food or water. Most bacteria die in the acidic stomach. However, some survivors reach the small intestine. They use tiny tails called flagella to swim through mucus.

Once they reach the intestinal wall, they stop swimming to save energy. The bacteria then release a cholera toxin. This toxin causes cells to release water, salt, and potassium. This leads to rapid dehydration and muscle cramps.

People have known about cholera for a very long time. Descriptions of the illness appear in Sanskrit literature from the 5th century BCE. In Europe, the name was once used for many stomach illnesses. It was not used for this specific disease until the early 19th century. A scientist named John Snow studied cholera in England. He worked between 1849 and 1854.

He used a map to show how water spread the germ. This was the first time anyone used mapping to track a disease.

There are many important facts about how cholera moves. It is spread mostly by unsafe water or food contaminated by human waste. Undercooked shellfish are a common source of the bacteria.

Cholera affects about 3 to 5 million people every year. It causes between 28,800 and 130,000 deaths annually. The most recent pandemic began in 1961 and continues today. Risk areas include Africa and Southeast Asia. Without treatment, the risk of death can be as high as 50%. With good treatment, that risk drops below 5%.

We can fight cholera using tools we use for other germs. Doctors use oral rehydration salts to replace lost fluids. These are slightly sweet and salty solutions.

Rice-based solutions are often the best choice. For children, adding zinc can also help them get better. People can also take vaccines by mouth for protection. In 2017, the FDA approved a vaccine called Vaxchora for certain adults. This helps protect people traveling to areas with active outbreaks.
