Maderno learned his craft in a very hands-on way. He began his career working in marble quarries in the north. Later, he moved to Rome in 1588 with four of his brothers. He worked as a marble cutter at first. This job helped him understand how to shape stone. This skill in sculptural work helped him design his buildings. He knew how to make stone look like it had rhythm. He used columns and shadows to make walls look deep and interesting.

Maderno's career grew through many important projects. In 1596, he finished the façade for the church of Santa Susanna. This was his first solo project and it was very confident. The front of the church used a mix of columns and decorations. This work caught the eye of Pope Paul V. The Pope then appointed Maderno as the chief architect of St. Peter's Basilica. Maderno had to change the plans made by the artist Michelangelo. He extended the building to create a long nave. He also added a grand façade with a balcony for Papal blessings. This part was completed in 1612.

He worked on many other famous buildings in Rome.

One major project was the church of Sant'Andrea della Valle. He designed the façade and a very large dome there. This dome is the third largest in Rome. He also worked on the interior of Santa Maria della Vittoria between 1608 and 1620. Maderno designed many palaces like the Palazzo Mattei. He even worked on the Quirinal Palace and the Palazzo Barberini. He also designed the base for a Marian column at Santa Maria Maggiore. His work was everywhere in the city.

Maderno's ideas influenced many artists who came after him. Even famous architects like Bernini and Borromini were influenced by his style. His work at the Palazzo Barberini helped shape the way they worked too. He also created a special space under the dome of St. Peter's called the Confessio. This area has 95 bronze lamps that stay lit all the time. Maderno died in Rome on January 31, 1629, at the age of 73. He was buried in the church of San Giovanni dei Fiorentini. His life changed the way the world looks at stone and light.
