Growing up in an atheist environment, she was raised without following a specific religion.

Akhtar's professional journey began with rigorous training and hands-on assistance in the film industry. She earned a filmmaking diploma from New York University (NYU). Before directing her own features, she served as an assistant to directors such as Mira Nair, Tony Gerber, and Dev Benegal. Her early roles were varied and foundational. She worked as a co-director for a music video titled "Price of Bullets" for the rock band Pentagram. She also served as a casting director for the 1999 drama "Split Wide Open" and the 2001 comedy-drama "Dil Chahta Hai."

Later, she acted as an executive producer for the 2007 film "Honeymoon Travels Pvt. Ltd."

Her career as a director officially launched in 2009 with the drama "Luck By Chance." This film followed the struggles of an actor attempting to enter the film industry. Although the film had an underwhelming performance at the box office, it received widespread critical acclaim for its screenplay and dialogue. This success earned Akhtar the inaugural Filmfare Award for Best Debut Director. In 2011, she achieved a major breakthrough with the comedy-drama "Zindagi Na Milegi Dobara." This film was a massive commercial success, grossing ₹1.53 billion (approximately US$19 million) worldwide. It ranked as the fifth highest-grossing Hindi film of that year and earned her a Filmfare Award for Best Director.

Akhtar has frequently collaborated with her longtime associate, Reema Kagti. Together, they co-wrote the psychological crime thriller "Talaash: The Answer Lies Within" in 2012. They also co-founded the Indian production company Tiger Baby Films in 2015. This company allows them to produce various projects, including the anthology films "Lust Stories" (2018) and "Ghost Stories" (2020). In these anthologies, Akhtar directed specific segments, such as the story of a 12-year-old girl in "Bombay Talkies" (2013) who aspired to be a Bollywood dancer. Her work often explores complex human relationships and social dynamics through different genres.

One of her most significant achievements occurred in 2019 with the musical drama "Gully Boy." This film was loosely inspired by the lives of Mumbai rappers Naezy and Divine. "Gully Boy" was a major commercial hit, grossing ₹2.38 billion (US$33.8 million) worldwide. This made it the ninth highest-grossing Hindi film of the year. The film also made history at the 65th Filmfare Awards by winning a record 13 awards for a single film. This broke the previous record of 11 wins held by the film "Black" (2005). Through this film, Akhtar became the only female director to win the Filmfare Award for Best Director twice.

In recent years, Akhtar has expanded her reach into the world of web series and international festivals. She co-created the romantic comedy-drama "Made in Heaven," which premiered on Amazon Prime Video in 2019. The series provides a dark look at the "Big Fat Indian Wedding" through the eyes of two wedding planners in New Delhi. In 2023, she co-created the crime thriller series "Dahaad." This series made history as the first Indian web series to premiere at the Berlin International Film Festival. She also directed the live-action adaptation of "The Archies" for Netflix in late 2023.

Akhtar's influence extends beyond the screen to the broader film community. She was invited to become a member of the Academy of Motion Picture Arts and Sciences. Her work continues to bridge the gap between traditional cinema and modern streaming platforms. Whether through large-scale musical dramas or intimate anthology segments, she remains a central figure in contemporary Indian storytelling. Her ability to balance commercial success with critical recognition defines her lasting impact on the industry.
