On February 1, 2003, a space ship named Columbia broke apart. 

The trouble began during the launch. Columbia used a large external tank for fuel. This tank was covered in insulating foam. The foam keeps the fuel cold. During the trip up, a piece of foam broke off. It was about 6 inches long and 6 inches wide. 
When Columbia came back to Earth, it got very hot. Hot gases went through the damage on the wing. This damaged the inside of the wing. The ship became unstable and broke apart. 
On February 1, 2003, the Space Shuttle Columbia broke apart during re-entry. 

The problem started during the launch from Kennedy Space Center. 

NASA engineers had seen foam break off before. In fact, foam strikes happened on 65 of the 79 missions that had video. Some engineers suspected the damage to Columbia was serious. However, NASA managers did not confirm the damage before the mission continued. When the shuttle re-entered the atmosphere, the damage became a major problem. Hot gases moved through the hole in the heat shield. This destroyed the internal structure of the wing. The orbiter became unstable and broke apart over Texas and Louisiana.

After the disaster, NASA stopped all shuttle flights for over two years.
The Space Shuttle Columbia disaster was a tragic event that occurred on February 1, 2003. During its re-entry into Earth's atmosphere, the orbiter disintegrated over Texas and Louisiana. 
The mission was primarily dedicated to scientific research. The crew used the SpaceHab module located inside the shuttle's payload bay for these studies. 
To understand the disaster, one must understand how the Space Shuttle was built. At launch, the shuttle stack consisted of the orbiter, two solid rocket boosters (SRBs), and a large external tank (ET). 

The thermal protection system was a complex multi-layered shield. The nose cone and wing edges used reinforced carbon–carbon (RCC), a composite material. Other areas used black high-temperature reusable surface insulation. For cooler parts, NASA used white low-temperature insulation or felt surface insulation. During re-entry, the TPS had to withstand temperatures as high as 3,000 degrees Fahrenheit. Meanwhile, it had to keep the orbiter's aluminum skin below 300 degrees Fahrenheit. This delicate balance was essential for the survival of the crew.
The failure began during the launch phase of STS-107. At 81.7 seconds into the flight, a piece of insulating foam broke off from the left bipod ramp on the external tank. 

When Columbia re-entered the atmosphere, the damage to the wing became fatal. The strike had allowed hot atmospheric gases to penetrate the heat shield. These gases entered the wing and destroyed the internal structure. This caused the orbiter to become unstable, leading it to break apart. 
The aftermath of the disaster led to major changes in how NASA operates. Flight operations were suspended for more than two years. Construction of the International Space Station (ISS) was also paused until July 2005.
