The trouble began during the launch. Columbia used a large external tank for fuel. This tank was covered in insulating foam. The foam keeps the fuel cold. During the trip up, a piece of foam broke off. It was about 6 inches long and 6 inches wide.

Sadly, all seven astronauts on board were killed. This was the second major disaster for the Space Shuttle fleet. The first happened in 1986 with the shuttle Challenger. This event changed how NASA manages space travel forever.

The problem started during the launch from Kennedy Space Center.

The foam struck the left wing of the orbiter. It hit the thermal protection system, which is a layer of heat shields. These shields protect the ship from the extreme heat of flying through the atmosphere.

NASA engineers had seen foam break off before. In fact, foam strikes happened on 65 of the 79 missions that had video. Some engineers suspected the damage to Columbia was serious. However, NASA managers did not confirm the damage before the mission continued. When the shuttle re-entered the atmosphere, the damage became a major problem. Hot gases moved through the hole in the heat shield. This destroyed the internal structure of the wing. The orbiter became unstable and broke apart over Texas and Louisiana.

The crew of STS-107 was a group of highly trained experts.

Rick Husband was the mission commander and a colonel in the Air Force. William McCool served as the pilot for the mission. Michael Anderson was the payload commander. Kalpana Chawla worked as the flight engineer. David Brown and Laurel Clark were mission specialists. Ilan Ramon was a payload specialist and the first Israeli astronaut. These seven people were working to conduct important scientific research.

After the disaster, NASA stopped all shuttle flights for over two years.

Work on the International Space Station also had to pause. Flights finally resumed in July 2005 with mission STS-114. NASA made many changes to make future trips safer. They began inspecting the heat shields while in orbit. They also made sure rescue missions were always ready to help. Most later missions flew only to the Space Station. This allowed the crew to use the station as a safe place if the shuttle was damaged.

The seven-member crew included Commander Rick Husband, pilot William McCool, and payload commander Michael Anderson. The team also included flight engineer Kalpana Chawla, mission specialists David Brown and Laurel Clark, and payload specialist Ilan Ramon. Ramon was notable as the first Israeli astronaut to fly in space. These experts were working to advance knowledge in various scientific fields during their time in orbit.

To understand the disaster, one must understand how the Space Shuttle was built. At launch, the shuttle stack consisted of the orbiter, two solid rocket boosters (SRBs), and a large external tank (ET).

The external tank held liquid hydrogen and liquid oxygen to fuel the Space Shuttle main engines (SSMEs). Because these liquids are extremely cold, the tank was covered in insulating foam. This foam prevented ice from forming on the outside of the tank. The orbiter itself was a winged vehicle designed to land like a glider. To survive the intense heat of re-entry, the orbiter used a thermal protection system (TPS).

The thermal protection system was a complex multi-layered shield. The nose cone and wing edges used reinforced carbon–carbon (RCC), a composite material. Other areas used black high-temperature reusable surface insulation. For cooler parts, NASA used white low-temperature insulation or felt surface insulation. During re-entry, the TPS had to withstand temperatures as high as 3,000 degrees Fahrenheit. Meanwhile, it had to keep the orbiter's aluminum skin below 300 degrees Fahrenheit. This delicate balance was essential for the survival of the crew.

The failure began during the launch phase of STS-107. At 81.7 seconds into the flight, a piece of insulating foam broke off from the left bipod ramp on the external tank.

This piece of foam was approximately 6 inches long and 6 inches wide. It struck the RCC panels on the left wing at a relative velocity of 522 feet per second. Although foam shedding had happened on 65 of the 79 missions with available imagery, some engineers suspected this strike was serious. However, NASA managers did not confirm the extent of the damage before the mission continued.

When Columbia re-entered the atmosphere, the damage to the wing became fatal. The strike had allowed hot atmospheric gases to penetrate the heat shield. These gases entered the wing and destroyed the internal structure. This caused the orbiter to become unstable, leading it to break apart.

The investigation revealed that the foam strike was a significant factor in the loss of the vehicle. The inability to inspect the damage in detail during the mission contributed to the outcome.

The aftermath of the disaster led to major changes in how NASA operates. Flight operations were suspended for more than two years. Construction of the International Space Station (ISS) was also paused until July 2005.

When flights resumed with STS-114, NASA implemented new safety protocols. They added on-orbit inspections to check the thermal protection system after ascent. They also ensured that rescue missions were always ready in case of damage. Most subsequent missions flew only to the ISS. This allowed the crew to use the station as a safe haven if the orbiter could not re-enter safely.
