He has shaped Albanian history through two major leadership roles. He served as the President of Albania from 1992 to 1997. Later, he served as the 32nd Prime Minister from 2005 to 2013. Today, he remains a central figure as a leader of the opposition. His career spans the transition of Albania from a communist state to a democracy. This journey involves complex political shifts, economic reforms, and significant legal challenges.

Before entering politics, Berisha focused on medical science. He was born in 1944 in northern Albania to a family of mountain farmers. He studied medicine at the University of Tirana and graduated in 1967. Berisha later specialized in cardiology, which is the study of the heart. He received a scholarship to study in Paris, France. He also earned a UNESCO fellowship for advanced training in Paris in 1978. During the 1970s, he became a leading researcher in Albanian cardiology. He even studied hemodynamics, which is the study of blood flow.

In the late 1980s and early 1990s, Berisha moved into political activism. He demanded more freedom of thought and expression for Albanians. He wanted citizens to have the right to travel freely. In 1990, he urged the government to abolish certain communist rules. He argued that true democracy required political pluralism, or having many different political parties. He helped found the Democratic Party of Albania. This was the first large opposition party in the country. During demonstrations, many members of this party wore white coats.

Berisha's presidency began with major economic and institutional changes. After the 1992 elections, he became the first non-Communist head of state in 53 years. He led efforts to privatize land and small state businesses. He also worked to stabilize the economy. Under his leadership, Albania moved from a three-figure inflation rate to a one-figure rate. The country also saw economic growth, reaching 9% in 1992. Albania joined the Council of Europe in 1995. However, his presidency faced a major crisis in 1997. Large investment schemes, known as Ponzi schemes, collapsed. These schemes had involved roughly $1 billion of citizens' savings. The resulting unrest led to a period of civil instability.

In recent years, Berisha has faced significant legal and political difficulties. He has been accused of involvement in corruption. Because of these accusations, the United States barred him from entering the country. The U.S. government cited concerns regarding public safety and organized crime. The United Kingdom has also applied sanctions against him. In 2021, he was expelled from the Democratic Party's parliamentary group. This caused a major split within the party between his supporters and those of Lulzim Basha.
