She has appeared in more than 200 films throughout her long career. While she has worked in many languages, she is most famous for her work in Hindi cinema. Rekha is celebrated for her ability to portray complex and strong female characters. These roles range from mainstream characters to literary figures and characters in independent films. Her career is defined by her capacity for reinvention. Even when her popularity declined, she found ways to sustain her status in the industry. This is the nation's fourth highest civilian honor.

She began her career as a child actress in Telugu films. Her first appearance was in the film Inti Guttu in 1958. She also appeared in the political satire Rangula Ratnam in 1966. Her first leading role occurred in the 1969 Kannada movie Operation Jackpot Nalli C.I.D 999. At age 13 or 14, she left school to pursue acting full-time. This decision was made to help her family during financial difficulties. She later expressed regret about not completing her formal education.

Moving to Bombay in 1969 marked a major turning point. Her Hindi debut was the film Sawan Bhadon in 1970. This film was a commercial success and established her as a rising star. However, her early years were filled with significant professional challenges. Many press critics were harsh regarding her physical appearance and weight. During her school years, she had experienced childhood obesity. Some classmates even bullied her using the Tamil word "lotta," meaning a metal pot. Instead of being discouraged, Rekha used this criticism as motivation. She worked intensely to improve her acting technique and her command of the Hindi language. This period of self-improvement resulted in a widely publicized transformation.

Her career entered a highly successful phase during the late 1970s. Performances in the films Ghar and Muqaddar Ka Sikandar brought her early recognition. Throughout the 1980s, she was one of the leading stars of Hindi cinema. She received her first Filmfare Award for Best Actress for the comedy Khubsoorat in 1980. During this time, she also explored parallel cinema. This movement consists of neo-realist arthouse films that focus on serious social themes. Her role as a classical courtesan in the film Umrao Jaan in 1981 was a major milestone. This performance earned her the National Film Award for Best Actress. She also helped lead a trend of women-centered revenge films, such as Khoon Bhari Maang in 1988.

As the decades progressed, Rekha continued to adapt her acting style. In the 1990s, she began taking roles that were different from her usual image. In the 1996 action thriller Khiladiyon Ka Khiladi, she played an underworld don. This role earned her a third Filmfare Award in the Best Supporting Actress category. In the 2000s, she transitioned into significant supporting roles and mother roles. She appeared in the 2001 dramas Zubeidaa and Lajja. She also acted in the science fiction film Koi... Mil Gaya in 2003. Her role in the 2006 superhero sequel Krrish became her highest-grossing release. This shows her ability to remain relevant across different genres of film.

Rekha's life outside of acting has also been significant. From 2012 to 2018, she served as a Member of Parliament for the Rajya Sabha. This is a house of the Parliament of India. Her personal life has frequently been the subject of media interest. There was much speculation regarding her professional pairing with actor Amitabh Bachchan. Her marriage to industrialist Mukesh Agarwal in 1990 lasted only seven months. He died by suicide shortly after their marriage ended. Because she is often reluctant to discuss her private life, she has sometimes been described as a recluse.
