Warburg’s scientific journey focused on cellular respiration, which is how cells use oxygen to create energy. During his time at the Naples Marine Biological Station between 1908 and 1914, he conducted vital early research. He studied the oxygen consumption of sea urchin eggs after fertilization. He discovered that the rate of respiration increases as much as sixfold during this process. He also found that iron is an essential element for the development of the larval stage. These studies helped establish his expertise in how living organisms manage metabolic processes.

His most significant work involved investigating the metabolism of tumors and the respiration of cells. Warburg focused specifically on cancer cells to understand why they behave differently than healthy cells. He discovered that animal tumors produce large quantities of lactic acid. This led to his famous cancer hypothesis regarding energy production. He proposed that cancer growth is caused by tumor cells generating energy, such as adenosine triphosphate (ATP), mainly through the anaerobic breakdown of glucose. This process is known as fermentation or anaerobic respiration.

In contrast to this, Warburg noted that healthy cells generate energy mainly through the oxidative breakdown of pyruvate. Pyruvate is a chemical end product of glycolysis, a process that happens in the cell. In healthy cells, pyruvate is oxidized within the mitochondria, which are the energy-producing parts of the cell. Because of this difference, Warburg hypothesized that cancer should be interpreted as a mitochondrial dysfunction. While modern science now views these metabolic changes as a result of mutations in oncogenes and tumor suppressor genes, his work remains a foundation for studying cancer metabolism.

Warburg's life was also shaped by his experiences during the First World War. He served as an officer in the elite Uhlan cavalry regiment. For his bravery during the war, he was awarded the Iron Cross (1st Class). This military service later helped him during the rise of the Nazi regime. Although he was labeled a "Halbjude" or "Mischling" due to his Jewish heritage, the Nazis allowed him to continue his research. They were interested in his focus on cancer, as Hitler himself was obsessed with the disease. Warburg's status as a decorated veteran also provided some protection during this period.

Despite the dangers, Warburg remained deeply devoted to his laboratory. He lived with his companion, Jacob Heiss, for 50 years. In 1943, he moved his laboratory to the village of Liebenburg to avoid air raids. He was known to be so focused on his work that he once advised a friend to think about nothing but science. Even after the war, he continued his research until the age of 86. He passed away in 1970 from a pulmonary embolism.
