Low back pain

life science Maturity 7-9 Next article →

Low back pain, also known as lumbago, is a very common medical condition. It affects the area between the lower edge of the ribs and the lower fold of the buttocks.

This region includes five lumbar vertebrae, labeled L1 through L5, and the sacrum. The condition involves the muscles, nerves, and bones of the lower back. It can feel like a dull, constant ache or a sudden, sharp sensation. Because it is so widespread, it is a major cause of disability and lost productivity worldwide.

Doctors classify low back pain based on how long the symptoms last. Acute pain is defined as lasting less than six weeks. Sub-chronic pain lasts between six and twelve weeks. Chronic pain is any pain that lasts for more than twelve weeks or three months. Most people find that their symptoms improve within a few weeks. In fact, between 40% and 90% of people recover within six weeks of the pain starting.

The structure of the lower back is quite complex. Between the vertebrae are fibrocartilaginous intervertebral discs. These discs have a gelatinous core surrounded by a fibrous ring. They act as cushions to prevent the vertebrae from rubbing together. They also help protect the spinal cord. Nerves pass through specific openings between the vertebrae to send sensory input and muscle messages. Stability is provided by the ligaments and muscles, such as the multifidus muscles. These muscles run along the spine to keep it straight and stable during walking or lifting.

Pain sensation happens through a specific four-step process. First is transduction, where sensory nerve cells convert a stimulus into an electrical signal. Second is transmission, where these signals travel through the spinal cord to the brain stem. Third is perception, where the brain processes these signals and gives them context. Finally, there is modulation, where the brain can increase or decrease the release of neurotransmitters to change the signal. In chronic pain, the pain processing system may malfunction. This can cause the body to feel intense pain in response to non-serious events.

There are several different types of causes for low back pain. Mechanical causes include muscle strains, spasms, or osteoarthritis. A herniated disc occurs when disc material pushes on a nerve root. This can cause sciatica, which is pain radiating down the legs. Non-mechanical causes include infections, such as osteomyelitis, or tumors. Inflammatory causes include conditions like ankylosing spondylitis. Other factors like obesity, smoking, and stress can also contribute to the problem. For women, conditions like endometriosis or uterine fibroids may cause pain.

Managing low back pain often begins with non-medication treatments. These include superficial heat, massage, acupuncture, or spinal manipulation. Doctors recommend continuing normal activity as much as the pain allows. If these methods are not enough, NSAIDs may be used. For severe cases, opioids are sometimes used, though they carry risks of addiction and overdose. Surgery is usually only considered for specific issues like spinal stenosis or disc-related disability. For many, counseling or antidepressants can help manage the mood changes that often accompany chronic pain.

Low back pain is a massive global health issue. Approximately 9% to 12% of people, or about 632 million individuals, have pain at any given time. About 40% of people will experience it during their lives, and this can reach 80% in developed countries. Most people first experience this difficulty between the ages of 20 and 40. Women and older people often report higher rates of pain and disability. In 2023, the World Health Organization identified lower back pain as a top medical condition. They noted that many people worldwide could benefit from improved rehabilitation programs.
