She was recognized for her ability to portray complex characters across film and television. Her career spanned several decades and included both leading and supporting roles. Fletcher achieved immense success, earning some of the most prestigious honors in the entertainment industry. She was particularly noted for her ability to play difficult or antagonistic characters with great skill.

Fletcher's upbringing in Alabama significantly influenced her life and her later work. She was born on July 22, 1934, in Birmingham to Estelle and Reverend Robert Capers Fletcher. Both of her parents were deaf and dedicated their lives to serving the deaf and hard-of-hearing community. Her father founded more than 40 churches specifically for the deaf in Alabama. Because Fletcher and her three siblings had normal hearing, they lived with an aunt in Texas for three months each year. This arrangement ensured they developed strong spoken English skills.

Her professional journey began with formal education and early television work. She earned a bachelor's degree in drama from the University of North Carolina at Chapel Hill in 1957. In the late 1950s, she appeared in various television series such as Lawman and Maverick.

She often found success in Westerns, a genre she felt suited her height. By 1960, she had made multiple guest appearances on the legal drama Perry Mason. These early roles provided the foundational experience necessary for her transition to major film productions.

Fletcher reached international fame through her performance in the 1975 film One Flew Over the Cuckoo's Nest. She portrayed Nurse Ratched, the antagonist and nemesis to the main character. To create this character, Fletcher drew upon her observations of how white people treated Black people in Alabama. This performance earned her the Academy Award for Best Actress, a BAFTA Award, and a Golden Globe Award. She was only the third actress in history to win all three awards for a single performance. During her Oscar acceptance speech, she used sign language to honor her parents.

Following her major success, Fletcher maintained a diverse and active career. She appeared in many films, ranging from the horror film Exorcist II: The Heretic to the drama Firestarter. Her filmography includes a wide variety of genres, including crime dramas and psychological thrillers. She also worked in television movies, such as The Karen Carpenter Story, where she played Agnes Carpenter. Even as she moved between different types of projects, she continued to demonstrate her range as a performer.

Television provided Fletcher with several long-term and recurring opportunities. From 1993 to 1999, she played Kai Winn Adami, a scheming Bajoran religious leader, on Star Trek: Deep Space Nine. She also received Emmy Award nominations for her guest roles in the series Picket Fences and Joan of Arcadia. Later in her career, she appeared in the series Shameless and her final role was as Rosie in the Netflix series Girlboss in 2017. Her ability to inhabit different worlds, from science fiction to gritty modern dramas, defined her professional versatility.

Fletcher's personal life was also marked by significant milestones. She married producer Jerry Bick, with whom she had two sons, John Dashiell Bick and Andrew Wilson Bick. She took an eleven-year hiatus from the acting profession to focus on raising her children. In 1982, she was recognized for her connection to the deaf community with an honorary degree from Gallaudet University. Fletcher passed away at her home in Montdurausse, France, on September 23, 2022, at the age of 88.
