Acting requires great skill and practice. Fletcher studied drama at the University of North Carolina at Chapel Hill. She graduated with her degree in 1957. Before her big movie roles, she worked on many TV shows. She appeared in shows like Lawman and Maverick in the late 1950s. She often played roles in Western stories because of her height. This early work helped her learn how to perform for cameras.

Fletcher's life was shaped by her family in Alabama. She was born on July 22, 1934, in Birmingham. Her parents were Estelle and Robert Capers Fletcher. Both of her parents were deaf. They spent their lives working with people who were hard of hearing. Because her children could hear, they lived with an aunt in Texas for part of the year. This helped them learn to speak English well. Her father even started more than 40 churches for the deaf.

Her most famous role came in 1975. She played Nurse Ratched in the film One Flew Over the Cuckoo's Nest. This character was a nemesis to the main character. Fletcher based this performance on how she saw people treated in her home state. The role made her a global star. She won an Academy Award, a BAFTA Award, and a Golden Globe. She was only the third actress ever to win all three for one single role. When she won her Oscar, she used sign language to thank her parents.

Fletcher continued to act for many decades. She played the religious leader Kai Winn Adami on Star Trek: Deep Space Nine from 1993 to 1999. She also appeared in shows like Picket Fences and Joan of Arcadia. She even had a role in the series Girlboss in 2017. Fletcher married producer Jerry Bick and had two sons. She took a break from acting for 11 years to raise them. She passed away in France on September 23, 2022, at age 88.
