Farrar began her musical education at the age of five in Boston. By age 14, she was already performing public recitals. Her training took her across several major cities in Europe and America. She studied voice with Emma Thursby in New York City and Paris. She later studied with the Italian baritone Francesco Graziani in Berlin. In Berlin, she also studied under the famed German soprano Lilli Lehmann. This training helped her make a sensation at the Berlin Hofoper in 1901. She debuted there as Marguerite in the opera Faust.

Her career in grand opera reached new heights in the United States. She debuted at the New York Metropolitan Opera on November 26, 1906. She performed the role of Juliette in the opera Roméo et Juliette. Farrar remained a member of the Metropolitan Opera company until 1922. During her time there, she sang 29 different roles. She completed a total of 672 performances for the company. Some of her most frequent roles included Madama Butterfly and Carmen. She performed Madama Butterfly 95 times and Carmen 58 times.

Farrar was also a pioneer in the world of silent films. Silent films are movies made without recorded speech or sound. Producer Jesse Lasky acquired her for Paramount in 1915. This was considered a sensational event in motion-picture history. She starred in over twelve films between 1915 and 1920. One of her most famous roles was in the 1916 film Joan the Woman. She played the character Joan of Arc in that film. Farrar personally considered this role to be her finest achievement. In 1916, she also won first place in a "Screen Masterpiece" contest.

Farrar's personal life often drew significant public attention. She had a seven-year relationship with the Italian conductor Arturo Toscanini. This relationship led to his resignation as principal conductor at the Metropolitan Opera in 1915. In 1916, she married the actor Lou Tellegen. Their marriage ended in a public divorce in 1923. Farrar was also a close friend of the famous tenor Enrico Caruso. It is said that Caruso gave her the motto "Farrar farà," meaning "Farrar will do it."

Maintaining such a busy career took a toll on her health. Farrar was known for her incredible work ethic. She gave between 25 and 35 performances every season at the Metropolitan Opera alone. This heavy workload caused her voice to decline prematurely. She retired from the opera stage in 1922 at the age of 40. Her final performance at the Met was as Zazà. After retiring from opera, she transitioned into concert recitals and radio broadcasts. She even served as an intermission commentator for the Metropolitan Opera radio broadcasts.

Farrar's legacy is preserved through many different forms of media. She recorded many songs for the Victor Talking Machine Company. In 1960, she was honored with two stars on the Hollywood Walk of Fame. One star recognizes her work in music and the other in film. She also published an autobiography in 1938 called Such Sweet Compulsion. This book used alternating chapters to tell her story. Farrar died of heart disease in 1967 at the age of 85. She is buried in Kensico Cemetery in Valhalla, New York.
