While these trains are highly efficient, they have been involved in serious incidents. On February 6, 2020, a train derailed at Ospedaletto Lodigiano. This event resulted in two deaths and 27 injuries. On December 10, 2023, a collision occurred at Faenza involving another passenger train. This incident injured 17 people. Additionally, on January 18, 2026, an ETR 1000 operated by Iryo derailed in Adamuz. This train collided with an Alvia Class 120 train. This specific accident caused 46 deaths across both trains. These events highlight the complexities of high-speed rail safety.
