He is widely recognized for his influential graphic novels and comic book runs. His work often features dark, gritty, and complex storytelling. Miller is particularly noted for his unique artistic style. He combined elements of American film noir with the visual influences of Japanese manga. He described this approach as a hybrid style. He felt American comics were often too wordy or constipated. Conversely, he felt Japanese comics could sometimes feel too empty. By blending these styles, he created a new visual language for the medium.

Miller's professional journey began with small assignments and significant mentorship. He moved to New York City to pursue his career in comics. He showed his art samples to the famous artist Neal Adams. Adams provided critique and occasional informal lessons to the young artist. Miller's first published work appeared at Western Publishing's Gold Key Comics imprint. He later worked for DC Comics, where he received a one-page war-comic job. His first confirmed credit was in Weird War Tales #64 in June 1978. He eventually moved to Marvel Comics, where he drew characters like Spider-Man. These early roles allowed him to build his technical skills as a professional.

One of Miller's most significant contributions was his revitalization of the character Daredevil.

When Miller began working on the title, sales were quite poor. He noticed the potential of a blind protagonist in a visual medium. With the help of editor Denny O'Neil, Miller began writing the series himself. Starting with issue #168 in January 1981, he took over full writing and penciling duties. This change caused sales to rise very quickly. Marvel began publishing the title monthly instead of bimonthly just three issues later. Miller transformed Daredevil from a second-tier character into one of Marvel's most popular heroes.

During his time on Daredevil, Miller introduced several iconic elements and characters. He created the ninja mercenary Elektra, who first appeared in issue #168. He also introduced the martial arts leader Stick, who served as a sensei. Miller added a complex mythology involving ninja clans, such as the Hand. The Hand was described as a league of ninja assassins using dark magic. He also introduced the villainous Kingpin of Crime. Miller's stories during this era were known for their darker themes. In one notable event in issue #181, the assassin Bullseye killed Elektra. This period of storytelling redefined the character's world and established Miller as a rising star.

Miller's success extended to creating original works and expanding other famous characters. He co-produced a popular miniseries for Wolverine, which helped expand that character's history. He also created his first creator-owned title, the six-issue miniseries Ronin, for DC Comics. In 1986, he released the landmark miniseries Batman: The Dark Knight Returns.

This story featured a 55-year-old Batman returning to fight crime in a dark future. It was printed in a prestige format using heavy-stock paper and squarebinding. This work helped usher in a new wave of adult-oriented storytelling in the industry. It remains a significant influence on how modern superhero stories are told.

Beyond comic books, Miller has had a successful career in the film industry. He wrote the scripts for the science fiction films RoboCop 2 and RoboCop 3.

He also shared directing duties for the film adaptations of Sin City. He produced the film 300 and directed the adaptation of The Spirit. His work on the Sin City film earned a Palme d'Or nomination. These projects show how his visual style translates from the printed page to the cinema screen. His ability to move between writing, drawing, and directing demonstrates his versatility as a creator.

Throughout his career, Miller has received nearly every major award in the comic book industry. In 2015, he was inducted into the Will Eisner Award Hall of Fame.
