Epilepsy is a group of brain disorders that cause a person to have seizures. A seizure is a sudden burst of abnormal electrical activity in the brain.

These episodes can cause many different symptoms. Some people might only have brief lapses in awareness. Others may experience muscle jerks or long convulsions. These events can sometimes lead to physical injuries like broken bones. A person is usually diagnosed with epilepsy after having two unprovoked seizures. These seizures must happen more than 24 hours apart.

Seizures work in different ways depending on where they start in the brain. The International League Against Epilepsy (ILAE) groups them into four main classes. Focal seizures start in just one area of the brain. These may involve localized networks or spread to nearby areas. A person might stay aware during a focal seizure. They might also experience an aura, which is a warning feeling.

Generalized seizures are different because they spread quickly across both sides of the brain. These usually make a person lose consciousness right away. They can include tonic-clonic seizures, which involve stiffening and jerking. There are also absence seizures that cause brief lapses in attention.

Doctors have studied these brain patterns for a very long time. The word epilepsy comes from Ancient Greek words meaning to seize or afflict.

Early thinkers like Hippocrates studied these conditions long ago. Today, doctors use many tools to find the cause of seizures. They use neuroimaging and blood tests to check the brain. They also use an EEG, which is a test that records electrical activity. This helps them rule out other conditions that look like seizures. Knowing the cause helps doctors choose the right way to help.

Many different things can cause epilepsy to happen. It can result from brain injuries, strokes, or infections. Tumors and genetic conditions are also common causes. In about 50% of cases, the cause remains unknown.

Around 50 million people worldwide live with epilepsy today. Nearly 80% of these cases occur in low- and middle-income countries. In 2021, epilepsy was responsible for about 140,000 deaths. This is an increase from 125,000 deaths recorded in 1990. Epilepsy is most common in both children and older adults.

Living with epilepsy can be a hard job, but there is much help available. About 69% of people can control their seizures with medicine.

These anti-seizure medications are often inexpensive and easy to find. If medicine does not work, doctors might suggest surgery or dietary changes. Some people even use neurostimulation to help their brains. Not all cases of epilepsy last a lifetime. Many people improve so much that they no longer need treatment. People may also wear medical alert bracelets to stay safe. This helps others know how to react if a seizure happens.
