Most economic activity involves small-scale farming, cattle ranching, and lumber. However, criminal enterprises like drug and human trafficking are also widespread.

Efforts to build a road have failed for many decades. Planning began in 1971 with American funding. This was halted in 1974 due to environmental concerns. Another attempt began in 1992. By 1994, a United Nations agency reported that a road would cause extensive environmental damage. Many groups oppose a road to prevent deforestation. They also want to protect indigenous cultures. One major concern is preventing foot-and-mouth disease. This disease could enter North America if a road were built. North America has not seen this disease since 1954.
The history of the region includes many significant events. The Guna people moved westward due to conflicts with the Spanish. In 1513, Vasco Núñez de Balboa reached the Pacific Ocean. 
In recent years, the Gap has become a major migration route. Since the 2010s, hundreds of thousands of people have traveled through it. Most migrants are from Venezuela and Haiti. They are traveling north toward the United States border. The number of people crossing has increased rapidly. In 2022, there were 250,000 crossings. In 2023, that number grew to more than 520,000 people. This is more than double the previous year's total. The lack of medical support and law enforcement makes this journey very perilous.
