The land here was shaped by the sea a long time ago. About 6,500 years ago, the sea reached its current level. The Bakkhali River and small streams from the hills filled the area with sediment. This created a flat plain where most of the city is built. Because much of the city is on a floodplain, it can be at risk from floods. This happens during big storms or cyclones. The weather is tropical and very wet during the monsoon season.

This place has a very long history with many different rulers. In the 9th century, kings from Arakan ruled the area. Later, the Mughals took control in 1666. A Mughal prince once camped here because the land was so beautiful. His group had one thousand palanquins with them. This created a place called Dulahazara, which means "one thousand palanquins." After the Mughals, many groups like the Portuguese and the British ruled the land. The city's name comes from a British officer named Captain Hiram Cox. He helped refugees find new homes in the area.
