Constipation is a bowel dysfunction rather than a standalone disease. It is a symptom that indicates the body is having difficulty moving waste. Most often, it is defined by infrequent bowel movements, usually fewer than three per week. People experiencing constipation may also deal with hard or dry stool. Other common symptoms include abdominal pain, bloating, and a sensation of incomplete evacuation.

This means the person feels as if they have not fully emptied their bowels. While common, constipation can lead to complications like hemorrhoids, anal fissures, or fecal impaction.

To understand how constipation works, we must look at the movement within the colon. Normally, the colon uses colonic propagating pressure waves to move contents forward. These waves are vital for normal defecation. When there are deficiencies in the frequency or strength of these waves, it can lead to severe defecatory dysfunction. If stool moves too slowly through the colon, it can become excessively hard and dry. This slow transit makes it much more difficult and painful to pass the waste. In some cases, the stool may appear as small, hard pellets.

Medical professionals categorize the causes of constipation into three distinct types. Primary or functional constipation is the most common form. This type is ongoing for more than six months and is not caused by an underlying medical condition or medication. In children, this might be caused by a reluctance to use the bathroom. In adults, it often stems from dietary choices or low physical activity. Secondary constipation is caused by other factors, such as medication side effects or metabolic disorders. Finally, congenital constipation is present at birth. This can involve structural anomalies like an imperforate anus or Hirschsprung's disease.

Many different factors can trigger these various types of constipation. Dietary habits play a massive role in how the colon functions. Dietary fiber is essential because it increases stool bulk while simultaneously softening it. A diet low in fiber or fluids can lead to primary constipation. Medications are another major secondary cause. For example, approximately 90% of people taking opioids develop constipation. Other drugs, such as certain antacids, calcium channel blockers, and anticholinergics, can also cause this issue. Calcium channel blockers like nifedipine can cause severe constipation by affecting motility in the rectosigmoid colon.

Medical conditions can also be a significant underlying cause. Endocrine and metabolic disorders, such as hypothyroidism and diabetes mellitus, are known to contribute. Some people may experience constipation due to neurological issues like Parkinson's disease. Structural causes can also create physical obstructions within the colon. For instance, colorectal cancer or strictures can create space-occupying lesions that stop the passage of stool. Even external masses from other malignancies can cause constipation through compression. In infants, Hirschsprung's disease is the most common medical disorder associated with this condition.

Doctors use specific tools to diagnose the problem and determine its severity. They often use the Rome IV Criteria to standardize a diagnosis across different age groups. This helps physicians define constipation in a consistent manner. A physical examination usually includes an abdominal exam and a rectal exam. An abdominal exam might reveal a mass if there is a significant stool burden.

An X-ray may be used if a doctor suspects a bowel obstruction. This imaging can reveal extensive impacted fecal matter within the colon. In more complex cases, a colonoscopy might be performed to check for tumors.

Treatment strategies depend on the specific cause and how long the symptoms have lasted. For many, simple lifestyle changes are the first step. Drinking enough fluids, increasing dietary fiber, and regular exercise can help normalize bowel movements. If these measures are not effective, doctors may recommend various types of laxatives. These include bulk-forming agents, osmotic agents, stool softeners, or lubricants. Stimulant laxatives are generally reserved for when other types do not work. In rare cases, more advanced treatments like biofeedback or surgery may be necessary.

Understanding constipation requires looking at the broader health of the body. It is a symptom that can signal many different systems are out of balance. For example, constipation can be a sign of anemia or significant weight loss in some patients. It can also be linked to systemic diseases like celiac disease. In the general population, constipation rates are between 2% and 30%. However, among elderly people living in care homes, the rate is much higher, at 50% to 75%. By identifying the root cause, healthcare providers can better manage the symptoms.
