It forms a large estuary south of Wilmington.

Nature works to shape this place. The land is made of barrier beaches. It also has silty outwash from the river. Two sweeping arcs of low beach meet here. Moving ocean currents help shape these arcs. These same currents create the Frying Pan Shoals. These shoals are part of the Graveyard of the Atlantic. Many plants grow in the sandy soil. Sea oats live on the upper dunes. Other grasses like saltmeadow cordgrass grow there too. These plants can live with salt.

History tells us how the name began. An explorer named Giovanni da Verrazzano arrived here in 1524. He was sailing for France. Later, a group led by Sir Richard Grenville arrived in 1585. His ship became stuck behind the cape. The crew feared they would wreck on the land. This fear gave the cape its name. It is the fifth-oldest English place name in the U.S. Other old names from 1584 include Roanoke Island.

Many important things happen at Cape Fear. The estuary drains a huge area of land. It covers the largest watershed in North Carolina. About 27% of the state's people live near this water. The area is also very important for weather. The National Hurricane Center uses the cape as a breakpoint. They use it for tropical cyclone warnings and watches. This helps keep people safe during big storms.

People have visited this coast for a long time. British General Sir Henry Clinton landed here in 1775. He came during the American Revolutionary War. The area is also known from movies. The film Cape Fear was made in 1962. There was also a remake in 1991. Both movies used this setting for their stories. It is a place where history and nature meet.
