His journey into theatre began with a single moment of inspiration. When he was eight years old, his aunt took him to a matinee performance of the musical Salad Days. This experience sparked a lifelong passion for the stage. He started his career at the very bottom of the industry. He worked as a stagehand at the Theatre Royal, Drury Lane. A stagehand is a person who works behind the scenes to manage equipment and scenery. He then moved up to become an assistant stage manager for various touring productions. By the 1970s, he was producing his own small tours in London.

Mackintosh's career is defined by several massive, record-breaking hits. In 1981, he produced the musical Cats, which was considered an unlikely subject at the time. It became a massive success and one of the longest-running musicals globally. Following this, he worked with the French writing team Claude-Michel Schönberg and Alain Boublil. Together, they brought the musical Les Misérables to the London stage in 1985. While it had a slow start, it became a massive hit through word-of-mouth. It is now the longest-running musical in West End history. In 1986, he produced The Phantom of the Opera, which remains one of the most commercially successful musicals ever made. This production is also one of the longest-running in London.

He has a specific way of making musicals successful on a global scale. Mackintosh was the first producer to recognize the value of worldwide touring. He realized that productions could be highly profitable in many different countries. This included markets in the former Eastern Bloc during the early 1990s. He also brought traditional theatre directors and technicians into the world of musicals. This helped raise the quality of musical productions. His company, Delfont Mackintosh Theatres, now owns eight different theatres in London. These include the Prince Edward, the Victoria Palace, and the Noël Coward theatres.

Some of his productions have faced challenges or controversy. The musical Miss Saigon, which opened in 1989, had very large advance ticket sales. However, it faced protests from Asian American actors regarding the casting of a Caucasian actor in a specific role. He has also seen less success with certain shows. Musicals like Moby Dick and Martin Guerre did not reach the same heights as his other hits. Even so, he continues to produce diverse works. He produced the stage adaptation of The Witches of Eastwick and a revival of the musical Oliver!. The 2008 production of Oliver! was reported as the fastest-selling show in West End history, with £15 million in pre-opening sales.

In recent years, Mackintosh has continued to bring major hits to the stage. He worked with Disney to produce the musical Mary Poppins, which ran in London and Broadway. He also brought the Broadway hit Hamilton to the West End in 2017. His influence is recognized by many official honors. In 1996, he was knighted for his services to musical theatre. In 2014, he became the first British producer to be inducted into Broadway's American Theater Hall of Fame. The Sunday Times Rich List estimated his net worth at £1.2 billion in 2021.

Beyond the theatre, Mackintosh has many different interests and responsibilities. He is the owner of the Nevis Estate in the West Highlands of Scotland. He also serves as a patron for The Food Chain, which is a charity based in London. His life connects the worlds of high-stakes business, international culture, and community service. Through his work, he has ensured that musical stories can be shared with audiences all over the planet.
