It is an autonomous region of Italy. The city serves as a vital cultural and economic hub for the Mediterranean. It is the 27th largest city in Italy. The city itself has about 150,000 inhabitants. The wider metropolitan area includes 69 other municipalities. This larger area has about 536,245 inhabitants.

The functional urban area reaches 476,975 people. This makes it a major center for politics, education, and industry.

The city's location provided many natural advantages for early settlers. It sits between the sea and a fertile plain. Two marshes surround the area to provide defense against inland attacks. Nearby high mountains offered a place for evacuation during times of danger. The city features a sheltered harbor that is one of the largest in the Mediterranean Sea. Natural resources also helped the city grow. These included salt from the local lagoons. The hinterland provided wheat from the Campidano plain. It also provided silver and other ores from the Iglesiente mines.

Human settlement in this area spans approximately five thousand years. This history begins in the Neolithic period.

The Phoenicians established a colony called Karaly around the 8th or 7th century BC. This settlement was located in the Stagno di Santa Gilla. Carthage later took control of the area in the late 6th century BC. This period saw significant growth, evidenced by the large Tuvixeddu necropolis. Later, the Romans took control in 238 BC after the First Punic War. The Romans built a new settlement called the vicus munitus Caralis. Over time, these two urban areas merged into one city.

During the Roman era, the city held great importance. It served as a chief naval station and the residence for the local praetor.

A Christian community was established there by the 3rd century AD. By the end of that century, the city had a Christian bishop. The city also saw various shifts in power during the Middle Ages. The Byzantine Empire ruled for a time, briefly interrupted by the Ostrogoths. Eventually, the independent Judicate of Cagliari was formed around the year 1020. This state included the Campidano plain and the Sulcis-Iglesiente region.

The Republic of Pisa exerted great influence over the region in the 11th century. Pisan merchants established a fortified city called Castel di Castro around 1216.

This fortified hill is the ancestor of the modern city district known as Castello. In 1258, the Pisans destroyed the old capital of Santa Igia. Following this, the Judicate of Cagliari was divided into three parts. The Crown of Aragon conquered Sardinia in the early 14th century. During the siege of Castel di Castro, the Aragonese built a stronghold on the hill of Bonaria.
