Born on April 4, 1932, in Manhattan, New York City, he came from a family connected to the arts. His father, Osgood Perkins, was a professional actor. Because his father was often away working, Perkins spent much of his early life with his mother, Janet Esselstyn. A French nanny named Jeanne lived with them, which helped Perkins become fluent in French.

This early environment shaped his personal development and his eventual path toward the stage.

Perkins's childhood involved significant emotional challenges. After his father died of a heart attack in 1937, Perkins experienced intense feelings of guilt. He later described feeling as though his private thoughts might have caused his father's death.

During his youth, he also faced difficult domestic circumstances. His mother began sexually abusing him, a behavior that continued into his adulthood. These experiences, alongside his father's absence, contributed to a sense of rebellion and academic struggle during his school years. He eventually attended Brooks School in Massachusetts, where he faced pressure to participate in activities like baseball despite a childhood stutter.

His professional journey began through summer stock theater.
