Analgesic

Analgesics are a group of medicines used for pain management. The word comes from Greek roots meaning "without pain."

While they are similar to anesthetics, they are actually different. Anesthetics temporarily reduce or stop sensation entirely. Analgesics focus on relieving pain. Some medicines are available over the counter in most countries. Others are prescription drugs because they carry risks. These risks include misuse, addiction, or the chance of an overdose. Using these drugs without medical supervision can be very dangerous.

Different medicines work in different ways to stop pain. One common type is paracetamol, also called acetaminophen. It is a mild analgesic used for mild to moderate pain and fever.

You can take it by mouth, rectally, or even through an IV. Its effects usually last between two and four hours. Another group is called NSAIDs, which stands for nonsteroidal anti-inflammatory drugs. These include aspirin, ibuprofen, naproxen, and diclofenac. They decrease pain and fever, and can also lower inflammation. Some NSAIDs are used as gels on the skin to avoid side effects in the rest of the body.

Scientists have found even more specific ways to target pain. They discovered a cyclooxygenase enzyme that has two versions, called COX1 and COX2. Traditional NSAIDs block both versions. However, researchers developed COX2 inhibitors to target only the COX2 enzyme. Drugs like celecoxib and etoricoxib do this. These were meant to cause less stomach bleeding. But researchers found that most drugs in this class increase the risk of heart events by 40% on average. This led to some drugs, like rofecoxib, being withdrawn from use.

Stronger medicines called opioids work on the brain's receptor system. Morphine is the most famous example of an opioid. Other types include codeine, oxycodone, and hydrocodone. These are very effective for severe pain, such as pain from cancer or surgery. However, opioids can cause side effects like nausea, vomiting, or constipation. They can also lead to tolerance. This means the body gets used to the drug over time. If this happens, the medicine might not work as well as it did before.

Doctors sometimes use other medicines to help the main pain reliever work better. These are called adjuvants. For example, some antidepressants are used to help with pain from nerve damage. Other drugs, called anticonvulsants, are also used for this purpose. These include medicines like gabapentin and pregabalin. Sometimes, doctors use topical medicines like lidocaine on the skin. This can numb a specific area, like a mouth sore, for a dental procedure. Using medicine on the skin can help avoid effects in the rest of the body.
