Death toll estimates vary significantly due to the chaos of the event. Some estimates range from 100,000 to 160,000 people. However, the Haitian government estimated the death toll reached as high as 316,000.

The earthquake was caused by complex movements deep within the Earth's crust. The region sits near the boundary where the Caribbean tectonic plate shifts eastwards against the North American plate. This movement occurs along the Enriquillo-Plantain Garden fault zone.

Following the main shock, the region experienced a continuous series of aftershocks. These are smaller earthquakes that occur in the same area after a large one. The U.S. Geological Survey recorded eight aftershocks within the first two hours. Within nine hours, 32 aftershocks of magnitude 4.2 or greater had been recorded.

By January 24, at least 52 aftershocks measuring 4.5 or greater had been documented. One particularly strong aftershock occurred on January 20. It measured 5.9 Mw and struck near the town of Petit-Goâve. This aftershock was strong enough to collapse seven additional buildings in that town.

Haiti has a long and documented history of seismic activity. The island of Hispaniola is seismically active, meaning it frequently experiences earthquakes. During the French colonial period, historian Moreau de Saint-Méry recorded significant damage. He noted that in 1751, nearly every masonry building in Port-au-Prince collapsed. A massive earthquake in 1770 also caused the entire city to fall.

Even the headquarters of the United Nations Stabilization Mission in Haiti (MINUSTAH) collapsed. This collapse resulted in many deaths, including the mission's chief, Hédi Annabi.

Infrastructure damage significantly hindered the early relief and rescue efforts. Communication systems, electrical networks, and hospitals were all severely damaged.

The Port-au-Prince seaport became unusable because a large container crane subsided due to weak foundations. The main road to Jacmel remained blocked by debris for ten days. This made it very difficult to deliver essential supplies to the southeast. At the airport, the control tower was also damaged. These logistical failures caused confusion and delays in getting medical care to survivors.
