Rehman's journey began with her talent for dance and a desire to support her family. She was born on February 3, 1938, in Chengalpet, Tamil Nadu. She was the youngest of four daughters in a Deccani Muslim family. As a child, she and her sisters trained in Bharatanatyam, a traditional Indian dance form. Although she originally dreamed of becoming a doctor, family financial circumstances and her mother's illness led her to pursue acting. She accepted movie offers that utilized her dancing abilities to help her family. Her debut occurred in the Telugu film Rojulu Marayi in 1955.

Her early career was shaped by significant collaborations and mentorships. She established a successful working relationship with actor Dev Anand, appearing in films like Solva Saal in 1958. During this time, critics noted her ability to handle both intense serious scenes and lighthearted moments. A major turning point came when filmmaker Guru Dutt became her mentor. He brought her to Bombay, now known as Mumbai, and cast her in the crime thriller C.I.D. in 1956. During this period, some suggested she use a stage name to appear more alluring. Rehman refused this suggestion and continued to use her birth name.

Rehman achieved widespread fame through her work in diverse dramatic roles. She collaborated with Dutt on several acclaimed films, including Pyaasa (1957), Kaagaz Ke Phool (1959), and Sahib Bibi Aur Ghulam (1962). The latter film was a major international success and received a nomination for the Golden Bear at the 13th Berlin International Film Festival. In 1965, she starred in the romantic drama Guide, based on a book by R. K. Narayan. Her role as Rosie, a strong-willed woman and dancer, broke many existing film stereotypes. This performance earned her her first Filmfare Award for Best Actress and is considered her signature role.

As her career progressed, Rehman reached new heights of commercial and critical success. She was paired with several major superstars, such as Dilip Kumar and Rajesh Khanna. In the late 1960s, she earned consecutive Filmfare Award nominations for her roles in Ram Aur Shyam (1967), Neel Kamal (1968), and Khamoshi (1969). She won her second Filmfare Award for her performance in Neel Kamal. In 1971, she won the National Film Award for Best Actress for her role in the crime drama Reshma Aur Shera. By the end of the 1960s, she had become the second-highest paid actress in Hindi films.

From the 1970s onward, Rehman transitioned into many significant supporting roles. She appeared in notable romantic dramas directed by Yash Chopra, such as Kabhie Kabhie (1976), Chandni (1989), and Lamhe (1991). She also worked in action films like Trishul (1978) and Mashaal (1984). After a period of absence from the industry, she returned for intermittent roles in social dramas. These included Water (2005), Rang De Basanti (2006), and Delhi 6 (2009). Her ability to adapt to different genres and age groups has defined her long-lasting presence in cinema.

Beyond her acting, Rehman is a dedicated philanthropist and social advocate. She works to support education and serves as an ambassador for RangDe, an organization that combats poverty in India.
