It sits in the Plaza de Oriente, directly opposite the Royal Palace. Many people call the building "El Real," which means "The Royal One." It is considered a top institution for performing and musical arts in Spain. It is also one of the most prestigious opera houses in all of Europe. The theatre serves as a central hub for grand musical performances and cultural traditions.

Inside the theatre, many moving parts work together to create a performance. The building has a floor area of 3,110 square meters. It features a stage that measures 210 square meters. The auditorium contains a maximum capacity of 1,958 seats. Because the building follows an old classical style, some seats have a limited view of the stage. To solve this, the theatre projects a live stream of the opera or ballet onto the upper side walls. This allows every audience member to follow the action clearly.

The theatre hosts a wide variety of artistic productions each year. From September to July, the season includes about seventeen opera titles. It also features two or three major ballets. These shows can be original productions or co-productions with other major international opera houses. The theatre also hosts many concerts and recitals. The resident ensemble of the theatre is the Orquesta Sinfónica de Madrid.

They have been the principal orchestra since 1998.

The history of the site began even before the current building existed. A former theatre called the Teatro de los Caños del Peral stood on the site. It opened in 1738 under King Philip V. That theatre was demolished in 1817 to make room for the new project. King Ferdinand VII began the groundbreaking for the current Teatro Real on 23 April 1818. Construction and planning lasted for thirty-two years.

Queen Isabella II formally opened the house on 19 November 1850.

Over the decades, the building has undergone several major changes. In 1925, the theatre closed because the construction of the Metro de Madrid caused damage to the structure. It eventually reopened on 13 October 1966 as a venue for symphonic music. This period saw the theatre host the Spanish National Orchestra. Later, a major refurbishment began on 2 January 1991. This project aimed to return the building to its role as an opera house.

The Teatro Real has a long history of hosting world-renowned artists. During the late 19th century, it featured performers like Adelina Patti and Francesco Tamagno. In 1917, the Ballets Russes of Diaghilev performed there with Nijinsky and Stravinsky. The theatre is also known for staging world premieres. Since its 1997 reopening, it has staged sixteen world opera premieres. These include works like "Don Quijote" by Cristóbal Halffter and "The Perfect American" by Philip Glass.

Beyond opera, the theatre is a vital part of Spanish public life. It has hosted the 14th Eurovision Song Contest in 1969. It also held the 25th Goya Awards in 2011. Since 2012, it has hosted the Lotería Nacional's special Christmas draw every year on 22 December. The building is officially listed as a Bien de Interés Cultural. This status has been held since 1993.
