The accident happened because of the cold weather. It was a very cold morning. The cold made rubber seals called O-rings too stiff. 
President Ronald Reagan formed a group to study the accident. They found that NASA did not fix known problems. They also did not listen to warnings about the cold. After this, NASA made new safety rules. They also built a new shuttle.

The Space Shuttle Challenger disaster was a tragic event in the history of space exploration. On January 28, 1986, the Challenger spacecraft broke apart during its flight. This mission was designated STS-51-L and was the tenth flight for the Challenger orbiter. It was also the 25th flight for the entire Space Shuttle fleet. The accident resulted in the deaths of all seven crew members on board. This was the first time an American crew died during a spaceflight. 
To understand the accident, one must understand how the shuttle was built. During launch, the shuttle stack consisted of three main parts. First was the orbiter, which was the winged vehicle carrying the crew and payload. Second was the external tank, or ET, which held liquid hydrogen and liquid oxygen. Third were the two solid rocket boosters, or SRBs, which provided most of the thrust at liftoff. The SRBs were manufactured by Morton Thiokol and were attached to the ET. 
The failure occurred within a specific part of the SRBs called a field joint. These joints connected the different segments of the booster together. To keep the joints sealed, engineers used two Viton-rubber O-rings. These O-rings were designed to create a double bore seal. They were meant to stop hot, high-pressure gases from escaping the booster. However, these O-rings were listed as Criticality 1R. This meant that an O-ring failure could destroy the vehicle and cause loss of life. 
The cause of the disaster was the failure of these O-ring seals. On the morning of the launch, the temperature was record-low. The cold weather caused the rubber O-rings to stiffen. Because they were stiff, they lost their flexibility and could not seal the joints properly. Shortly after liftoff, the seals were breached. Hot, pressurized gas leaked through the joint in the right SRB. This gas burned through the aft attachment strut that connected the booster to the external tank.
This leak caused a chain reaction that destroyed the spacecraft. The hot gas burned into the external tank itself. This caused the internal structures of the tank to collapse. As the tank failed, the rotation of the SRB caused the shuttle stack to travel at Mach 1.92. At this high speed, aerodynamic forces tore the orbiter apart. The crew compartment was separated from the rest of the shuttle. The impact of the compartment with the ocean surface was not survivable. 
Following the accident, President Ronald Reagan created the Rogers Commission to investigate. The commission looked into why the disaster happened and how to prevent it. They found that NASA's organizational culture and decision-making processes were flawed. Engineers had warned that low temperatures were dangerous for the O-rings. However, NASA managers did not report these technical concerns to their superiors. The commission also noted that test data from 1977 had shown flaws in the SRBs.
The disaster led to major changes in how NASA operates. The Space Shuttle program entered a 32-month hiatus. NASA established the Office of Safety, Reliability, and Quality Assurance to improve safety. They also began using expendable launch vehicles to deploy commercial satellites. A new orbiter was approved in 1987 to replace the Challenger. This new orbiter first flew in 1992. Future missions used redesigned SRBs and required crews to wear pressurized suits.
