He brought sacred images with him to prove his right to rule. This move was intended to unify the Lanna and Lan Xang realms. However, the nobles of Lanna eventually chose a different relative to lead them in 1551. Setthathirath spent several years fighting to regain control of his territories.

In 1560, Setthathirath made a decision that changed the region forever. He moved the capital of Lan Xang from Luang Prabang to Viangchan. This city would serve as the capital for the next 250 years. To support this move, he started a massive building program. He built a large formal palace and strengthened the city defenses. He also constructed the Haw Phra Kaew to house the Emerald Buddha. Other projects included renovating the Pha That Luang and building Wat Xieng Thong. These efforts made Viangchan a powerful center of culture and defense.

Much of his reign was defined by wars with the Burmese Empire. The Burmese conqueror Bayinnaung had already taken Xieng Mai and Ayutthaya. Setthathirath used clever military tactics to defend his land. He organized guerrilla warfare, which involves small, mobile groups using surprise attacks. These raids harassed Burmese troops and caused them to suffer from disease and hunger. In 1565, the Burmese were forced to retreat from Lan Xang. In 1570, Setthathirath led a successful counterattack. During this battle, his forces took 100 elephants and 30,000 prisoners from the retreating Burmese.

Setthathirath also attempted to expand his influence through diplomacy and conquest. In 1563, he signed a treaty with the kingdom of Ayutthaya. This alliance was sealed by a marriage between Princess Thepkasattri and the Ayutthayan leadership. However, the alliance faced many challenges due to Burmese interference. Setthathirath also led armies against the Khmer Empire in 1571 and 1572. He hoped to gain access to the sea and trade for European firearms. During the second invasion, a Khmer king was killed in an elephant duel. Despite these efforts, the Khmer were able to rally and force a retreat.

His life ended suddenly in 1571 due to a conspiracy. A group led by Lord Phya Nakhon and a former abbot killed him. He was only 37 years old when he died. Because his heir, Prince Noi Hno Muang Keo Koumane, was only a toddler, the country fell into chaos. For many years, different leaders fought for power. The Burmese eventually conquered the country in 1574. The young prince was taken to Burma as a captive. It took many years before the prince was released and returned to rule in 1591. His return helped the people begin the long process of regaining independence.
