The sacrum features three distinct surfaces shaped for the surrounding anatomy. The pelvic surface is concave from the top and curves slightly side to side. It has four transverse ridges marking where the original vertebrae once separated. At the ends of these ridges are four anterior sacral foramina. These holes allow sacral nerves and arteries to pass through.

The sacral promontory marks part of the border of the pelvic inlet. It forms a 30-degree sacrovertebral angle with the last lumbar vertebra.

Medical science recognizes several conditions related to this bone. Spina bifida is a congenital disorder caused by a defective embryonic neural tube. This results in an incomplete closure of the vertebral arch or the spinal canal. Another condition is caudal regression syndrome, also called sacral agenesis. This involves an abnormal underdevelopment of the lower spine during the seventh week of pregnancy. Additionally, the sacrum is a site for chordomas. These are sarcomas that grow from remnants of the embryonic notochord.

The sacrum is not unique to humans, as other vertebrates share similar structures. In dogs, the sacrum is formed by three fused vertebrae. A horse has a sacrum made of five fused vertebrae. Even rats have four pelvic vertebrae that fuse together. In upright vertebrates, the sacrum can move slightly along the sagittal plane. This movement is called nutation when the sacrum rotates upward and forward. The reverse motion is known as counter-nutation.
