The city's importance comes from its unique physical setting. It sits at the intersection of two major routes. These are the Route E35/A1, which connects Bologna and Milan, and the Route E70/A21, which connects Brescia and Turin. Piacenza is also located at a confluence. This is a place where two rivers meet. The Trebbia River drains the northern Apennine Mountains into the area. The larger River Po drains toward the east.

The etymology, or the history of the name, is very old. It comes from the Latin word "Placentia." This word is derived from a verb meaning "to please." Some etymologists have translated the name to mean "pleasant" or "comely abode." This name was originally given as a good omen for the settlement. The city's culture is a mix of different influences. It integrates characteristics from the nearby Ligurian and Piedmontese territories. It also has a strong Lombard influence due to its constant communication with Milan.

Before the Romans arrived, the land was occupied by different groups. The Ananes, a tribe of Cisalpine Gauls, lived on the right bank of the Po River. Before the Gauls, the Etruscans inhabited these plains. The Romans officially founded Piacenza in May 218 BC. They established it as a Roman military colony. The Senate wanted to build it quickly after a war with the Gauls ended. They gave colonists only 30 days to arrive at the site. Each colony was meant to be settled by 6,000 Roman citizens. These citizens were given Latin Rights, which gave them specific legal statuses.

History was often difficult for the early colonists. The local Gauls drove the Romans off the land. The colonists had to seek help from the city of Mutina. Eventually, the Senate sent two legions under Gaius Atelius to help. They successfully built a castra, or military camp, to support the city. Piacenza had to be walled almost immediately for protection. In 209 BC, Hasdrubal Barca crossed the Alps and tried to siege the city. He failed and withdrew. However, the Gauls later sacked the city in 200 BC. They burned the city and sold the people into slavery. The Romans later recovered 2,000 of those citizens.

During the Middle Ages, Piacenza grew into a very wealthy city. It became a free commune and joined the Lombard League. This helped the city expand its territory against neighbors like Cremona and Pavia. Agriculture and trade flourished during this time. The city's wealth was shown through new buildings and urban planning. In the 17th century, a coin was even made to celebrate this success.
