Smith’s musical process often involves a blend of melody and spoken word. In 1975, she released her debut album, Horses. The album was produced by John Cale. This method of combining text and music became a signature of her style.

Her career includes several distinct musical eras and groupings. In 1973, she formed the Patti Smith Group. This band included Lenny Kaye, Richard Sohl, Ivan Král, and Jay Dee Daugherty. The group recorded their first single, "Hey Joe/Piss Factory," in 1974. The B-side of this single described her feelings of alienation while working on a factory assembly line. Later, the band released the album Radio Ethiopia, which had a rawer sound influenced by the band MC5.

Smith has also shown great resilience during difficult times. In January 1977, she suffered a serious injury after falling 15 feet from a stage in Tampa, Florida. The fall broke several cervical vertebrae, which are bones in the neck. She used the resulting period of rest and physical therapy to reorganize her life.

She returned to music and continued to release albums like Dream of Life in 1988 and Gone Again in 1996. Her ability to continue creating after such an event is a notable part of her history.

Her work connects deeply to broader cultural and social themes. She has collaborated with diverse artists, such as Michael Stipe of R.E.M. and the band U2. Her songs often address social issues, such as the 1959 invasion of Tibet or the death of Kurt Cobain.

She also uses her platform to honor others, such as during her 2006 performance at CBGB. In that show, she read a list of punk rock musicians and advocates who had died. This connection to the history of her community remains a central part of her legacy.
